Peripheral Vision/Confrontation
Patient focus on object far away with symptomatic eye closed.
Have pt say ‘now’ when they see the object (S-50, M-60, I-70, L-90).
Repeat on other eye.
Any pain?
Pupillary Light Reflex
Dim the light.
Pt look at distant object and divide eyes with hand/notecard.
Shine light 2-4x in asymptomatic eye.
Repeat on symptomatic side.
Any pain?
Accommodation
Find distant object for pt to look at.
Have patient follow object with eyes only.
Instruct patient to look at distant object when you say ‘now’.
Any pain?
Corneal Light Reflex
Stand at patient’s midline.
Have patient look at your glabella.
Shine light at nasal bridge.
Examine reflection in both eyes - should be mirrored.
Any pain?
Cardinal Planes of Gaze
Have patient follow point of pen with their eyes only – not moving their head.
Perform H or star pattern.
Look for nystagmus/abnormal movement.
Perform on other side, ask if pain.
SCM & Trap Test
Test traps – have patient shrug/elevate their shoulders, then do the same against resistance.
Test SCM: if symptoms on R side, have patient turn head to R (test L SCM).
Repeat on other side.
Any pain?
Muscle Stretch Reflex - DTR
Patient seated with eyes closed.
Tap on muscle/tendon.
Grade reflex.
Test other side.

Adam’s Position
Pt standing, doctor behind.
Pt flexes forward to try and touch floor - bend at waist.
Any pain?
Beevor’s Sign
Pt supine
Pt does partial sit-up while dr looks at umbilicus.
Pt lifts legs off table, check umbilicus.
Any pain?
Chest Expansion
Pt seated.
Measure chest diameter and 4th ICS.
Pt fully exhale - measure.
Pt fully inhale - measure.

Rib Motion Test
Pt supine
Females - lift tissue UP
Dr place hands on lower rib cage.
Pt inhale, then exhale
Females - pull tissue DOWN
Dr place hands on upper ribcage.
Pt inhale then exhale.

Pt standing, doctor stands behind
Pt actively flexes to each side.
Dr observe for normal lateral flexion & contraction of paraspinals.

Schepelmann’s Sign
Pt seated or standing
Dr stand behind pt, observe pt’s back.
Pt active raise arms over head.
Pt laterally flex to each side.
Any pain?
Straight Leg Raiser
Pt lie supine.
Dr raise leg on side of pain until pain is produced/increased.
Note angle pain is produced.

Pt lie supine.
Dr raise leg on OPPOSITE side of pain until pain is produced/increased.

Pt lie supine.
Dr raise leg on side of pain to point pain is produced, then slightly lower.
Pt tell dr when pain decreases.
Apply sharp dorsiflexion.

Dr raise leg on OPPOSITE side of pain to point pain is produced, then slightly lower.

Dr place slight pressure down on head.

Dr push down prox to knee while pt is extending leg at hip.

Pt hold position for 5-10 seconds

Dr passively raise legs to 90 degrees.

Lasegue’s Test - Kernig’s
Pt supine
Dr place 1 hand on ankle and 1 on knee.
Dr passively flex hip to 90 and knee to 90.
Dr slowly extend pt’s knee to straight up.
Repeat on other side.

Dr flex asymptomatic hip/knee to 90.
Try to completely extend leg.

Pt supine, knees straight.
Dr passively flex pt’s head.

Pt walk on heels.

Kemp’s Test
Pt seated, dr supporting from behind.
Pt lean forward & to side opposite side being tested.
Dr circumducts pt’s trunk toward affected side.
Dr rotates, laterally flexes, & extends pt’s trunk to side being tested – stick stomach out.

Pt standing.
Pt squat and bounce twice.
Pt stand.
Have pt perform 5x.
