Define IUFR
Inability of fetus to attain full genetic potential
Small for gestational age & low birth weight babies fall under which percentile?
<10th percentile for GA
Usually weight for low birth weight?
<2.5kg
Usually weight for very low birth weight ?
<1.5kg
Clinical signs of IUGR?(7)
Inadequate fundal growth
Fetal head hard & larger than expected
Decreased amniotic fluid index (oligohydramnios)
Increased flexion of fetal body
Decreased fetal movement
Cedric more favourable than expected for GA
Increased variable decelerations on CTG
How do we diagnosis IUGR clinically?(3)
Two successive SFH <10th centile
Any 3 values <10th centile
3 values forming a plateau irrespective of whether it’s below 10th centile

When IUGR is suspected what can we confirm and evaluate?(4)
Confirm GA
Confirm FH present
Evaluate the abdomen and cervix
Exam signs of chronic disease
On ultrasound what will we look for? (3)
Fetal growth > symmetrical vs asymmetrical
Amniotic fluid index
Doppler > umbilical artery, middle cerebral artery

What are the pre-placental causes of IUGR (maternal) ? (7)
Living at high altitude
Maternal cyanotic heart lesions
Severe maternal respiratory distress
Chronic malnutrition
Renal disease
Maternal sickle cell anemia
Tobacco, alcohol, cocaine
What are placental causes of IUGR?(4)
Inadequate trophoblastic invasion > hpt disease
Chronic partial placental separation
Extensive placental infarction
Chorioamgioma
What is the post-placental (fetal) causes?
Post-term pregnancies > placenta cannot adequately supply foetuses needs
What investigations can be done to diagnose IUGR?(2)
Amniocentesis
TORCH
Maternal management for IUGR?(3)
Monitor for pre-eclampsia toxaemia (PET)
Quit smoking
Aspirin
What are the fetal managements of IUGR ? (3)
U/S > amniotic fluid index + Doppler
If growth plateaus <34 weeks > steroids 2x weeks, consider admission
If >34 weeks normal Doppler consider weekly surveillance
What are the consequences of poor placental function during labour & delivery?(2)
Fetal distress
Meconium stained liquor
What are the consequences of oligohydramnios during labour & delivery?(3)
Fetal lung hypoplasia
Fetal pressure abnormalities
Variable decelerations on CTG
What are the consequences of meconium leaking during labour & delivery?(2)
Meconium aspiration and asphyxia
Intrauterine infection
Define IUFD
Death of baby in utero >- 500g presents with decreased/ absent fetal movements confirmed on ultrasound

What is the management of IUFD?(7)
Inducing labour with medication
Wait for labour to occur naturally within a week / 2
Grief counselling
Look fir cause (X-ray baby, examine placenta)
Print pictures of baby
Postnatal follow-up appointment
