leukemia associated gingivitis
-malignancy of blood forming tissue
-leukemic cells can infiltrate gingival tissues
-acute (childhood), chronic (adults)
-usually local irritants such as biofilm initiate the oral changes
-immature/abnormal WBC infiltrate gingival CT via blood vessels & stimulate tumour-like enlargement
-stimulates fibroblasts, increases gingival thickness and creates pockets
-may get purplish masses that develop
types of leukemia
-myeloid, lymphoid
-acute, chronic
planning care for leukemia-associated gingivitis
-medical cons regarding blood studies prior to any invasive assessment or treatment
-prophylactic antibiotics may be required
-client education, debridement, correction of biofilm retentive factors
-eval
-prevention
-frequent maintenance
leukopenia
-large group of disorders that affect the cellular elements of the blood
gingival diseases associated w/ blood dyscrasias
-aplastic anemia, agranulocytosis, cyclic neutropenia, various forms of leukemia
planning care for gingival diseases associated w/ blood dyscrasias
-cons w/ the client's physician to determine risk of infection
-premed w/ antibiotics prior to invasive treatment if WBC low
-consult physician re type of premed
-proceed w/ perio therapy, regular maintenance intervals when cleared
-may be dealing w/ undiagnosed systemic disease
-oral signs may be the first of the disease
what should we do if we suspect a systemic disease or the clinical signs are more than one would expect for the amount of biofilm and/or deposit
-medical consult
