Notice of Claim
Under the Notice of Claim provision, the insured has 20 days to notify the insurer of a loss.
Claim Forms
After receiving a notice of claim, the insurer must provide claim forms to the insured within 15 days.
Proof of Loss
The insured must provide written proof of loss within 90 days of the loss.

An insurance company in Ohio must acknowledge the receipt of a claim within 15 days.

Indemnities for any loss under health policies in Ohio must be paid within 30 days after the insurer receives valid proof of loss.
Ongoing Disability Notice
If an insurer is paying loss-of-time (disability) benefits for at least 2 years, they can require notice of continued disability every 6 months.
Legal Actions
An insured must wait 60 days after filing proof of loss before they can file a lawsuit against the insurer.

An insured can bring a legal suit against the insurer within 3 years of submitting proof of loss.

Insurers in Ohio have 21 days to accept or reject a claim after receiving valid proof of loss.
