CN III (3) – Oculomotor
Check for PERRLA and ptosis (drooping eyelid).

Perform the 6 cardinal fields of gaze to follow your finger.
CN VII (7) – Facial
Ask the patient to smile, raise eyebrows, and close eyes tightly.
CN VIII (8) – Vestibulocochlear
Conduct the whisper test or finger-rub test.
CN IX (9) – Glossopharyngeal
Instruct the patient to say 'Ahhh' and observe the uvula rise.
CN X (10) – Vagus
Instruct the patient to say 'Ahhh' and ensure the soft palate rises and uvula remains midline.
CN XI (11) – Accessory
Assess by having the patient shrug shoulders and turn head against resistance.
