• treatment/prevention of osteoporosis in post-menopausal women:

  • 60 mg qd WITHOUT regards to meals OR time of day

Raloxifene HCl (Evista)

• broad treatment of various bacterial infections (due to gram ± bacteria) in Adults:

  • 250 to 500 mg q8h

  • 500 to 875 mg q12h

Amoxicillin Trihydrate (Amoxil, Moxatag, & Trimox)

• treatment of sinusitis, otitis media, lower respiratory tract infection, SSSI, or UTI (due to beta-lactamase producing bacteria) in Adults with MORE severe infections:

  • 500/125 mg q8h

  • 875/125 mg q12h

Amoxicillin with Clavulanate Potassium (Augmentin & Augmentin XR)

• treatment of mild to moderate lower/upper respiratory tract infection or SSSI:

  • 500 mg as a SINGLE dose on the 1st day followed by 250 mg qd x 4 days

• treatment of acute exacerbation of chronic bronchitis in Adults:

  • 600 mg qd in 1 OR 2 doses

  • 150 to 450 mg q6h

Clindamycin HCl (Cleocin)

• treatment of various bacterial infections (due to gram ± bacteria):

  • 100 mg q12h

Doxycycline Hyclate (Vibramycin & Doryx)

• treatment of acute bacterial exacerbation of chronic bronchitis:

  • 500 mg q24h x 7 days

Levofloxacin (Levaquin)

• treatment of various bacterial infections (due to gram ± bacteria):

  • 200 mg INITIALLY followed by 100 mg q12h

Minocycline HCl (Minocin, Dynacin, & Solodyn ER)

• topical treatment of impetigo:

  • cover the affected area w/ a SMALL film tid

Mupirocin (Bactroban)

• treatment of ophthalmic infection associated w/ conjunctivitis:

  • 1 to 2 gtt(s) into the conjunctival sac(s) q2-4h wa x 2 days THEN 1 to 2 gtt(s) into the conjunctival sac(s) qid x 5 days

Ofloxacin (Ocuflox)

• treatment of UTI, shigellosis, or acute exacerbation of chronic bronchitis in adults:

  • 800/160 mg q12h x 10 to 14 days

Sulfamethoxazole with Trimethoprim (Bactrim & Septra)

• adjuvant treatment of hormone receptor-positive early breast cancer in post-menopausal women:

  • 1 mg qd

Anastrozole (Arimidex)

• treatment of metastatic breast cancer in men/post-menopausal women; patients whose tumors are estrogen-receptor positive are more likely to benefit from this therapy:

  • 20 to 40 mg qd given in 2 div doses (am & pm)

Tamoxifen Citrate (Nolvadex)

• symptomatic control of severe, recalcitrant, disabling psoriasis that is NOT adequately responsive to other forms of therapy:

  • 10 to 25 mg qw until adequate response is achieved

Methotrexate (Trexall)

• dosages must be individualized:

  • range from 0.75 to 9 mg qd, depending on the disease being treated

Dexamethasone (Decadron)

• dosages must be individualized:

  • range from 4 to 48 mg qd

Methylprednisolone (Medrol)

• dosages must be individualized:

  • range from 5 to 60 mg qd

Prednisone (Deltasone)

• treatment of acute uncomplicated UTI:

  • 100 mg q12h x 7 days

Nitrofurantoin (Macrodantin, Macrobid, & Furadantin)

• symptomatic/asymptomatic trichomoniasis 7 day therapy:

  • 250 to 500 mg tid x 7 days

Metronidazole (Flagyl)

• treatment for hypertension:

  • max dose 10 mg qd

• treatment for hypertension:

  • 1 cap qd

    • lowest dose favorable to avoid peripheral edema

• treatment of hypertension (SR capsule):
