AP projection (External)
Upright or Supine
Center shoulder to midline of grid
Center IR at 1inch below Coracoid process
External Rotation Humerus
Supinate Hand
Epicondyles need to be parallel with IR
Suspend Respiration
CR needs to be perpendicular 1inch below coracoid process

AP Shoulder (Internal Rotation)
Center Shoulder Joint to the midline of Grid
Center the IR to 1inch below the the coracoid process
Internal Rotation
Flex elbow, rotate arm internally so back of hand is against hip
Places epicondyles perpendicular to the plane of the IR
Suspend Respiration
CR perpendicular to 1inch below coracoid process
Can be palpitated inferior to the clavicle and medial to the head

AP Rotation (Neutral Rotation)
Center the shoulder joint to the midline of Grid
Center the IR 1inch below the coracoid process
Neutral Rotation
Place palm of hand against hip
Roll the arm internally so that epicondyles are at a 45 degree angle with the plane of the IR
Respiration Suspended
CR perpendicular to the IR at 1inch below the clavicle
Can be palpitated inferior to the clavicle and medial to the humeral head

Structures shown: Bony and soft structures of the shoulder and proximal humerus. Scapulohumeral joint relationship is shown
External Rotation
Greater Tubercle in profile on lateral aspect of Humerus
Scapulohumeral joint visualized with slight overlap of humeral head on glenoid cavity
Glimpse of lesser tubercle between humeral head and greater tubercle
Internal Rotation
lesser turbercle in profile and pointing medially
Glimpse of greater tubercle superimposing the humeral head
Greater overlap of humeral head and glenoid cavity
Neutral Rotation
Greater tubercle partially superimposed the humeral head
Humeral head in partial profile
Slight overlap of humeral head on glenoid cavity
