Stress: Stress can diminish concentration and increase risk of injury.
Mental Health: Conditions like depression or confusion can lead to reduced awareness of surroundings.
Family Support: Assess the availability of support systems in the patient’s home.
Self-Care: Determine if the patient can care for themselves and identify barriers if they cannot.
Assessments should include:
Level of Consciousness: Evaluate whether patients are alert, drowsy, stuporous, or in a coma.
Mobility and Gait Status: Determine if patients have an unsteady gait or other mobility issues.
