Risk for Falls

  • All patients are considered at risk for falls due to various reasons.

Factors Contributing to Fall Risk

  • Medications: Certain medications may increase fall risk.

  • Limited Mobility: This can significantly heighten the likelihood of falls.

    • Interventions should include creating a safe environment and ensuring availability of assistive devices (cane, walker, wheelchair).

Psychological Considerations

  • Stress: Stress can diminish concentration and increase risk of injury.

  • Mental Health: Conditions like depression or confusion can lead to reduced awareness of surroundings.

Home Environment Factors

  • Family Support: Assess the availability of support systems in the patient’s home.

  • Self-Care: Determine if the patient can care for themselves and identify barriers if they cannot.

Anticipating Discharge Needs

  • Assessments should include:

    • Level of Consciousness: Evaluate whether patients are alert, drowsy, stuporous, or in a coma.

    • Mobility and Gait Status: Determine if patients have an unsteady gait or other mobility issues.
