    • It is vital to understand which problems are most detrimental to the client's wellbeing and recovery.

Goal Formulation

  • Immediate and Long-Term Goals: Work with the client to establish clear, achievable objectives.
    • Immediate goals may focus on short-term outcomes, while long-term goals address deeper, ongoing issues.
  • Identify Treatment Methods: Decide on effective methods and resources that will be utilized to meet the agreed-upon goals.
    • These should be evidence-based and tailored to the client’s unique situation.

Treatment Plan Requirements

  • Assessment Basis: The treatment plan must derive from the comprehensive assessment of the client.
  • JAHCO Requirements:
    • Treatment plan must be developed based on intake information and must be completed within 72 hours following the initial intake.
    • Adherence to guidelines set forth by JAHCO is critical for accreditation and insurance purposes.
