267- Generate Insights

Purpose of Analysis Phase

  • Examine gathered data to uncover root causes of observed issues.
  • Key question: “Why did this problem occur?” to enable actionable insights.

Brainstorming

  • Gather team; share ideas rapidly to surface potential causes.
  • Common formats:
    • Free-for-all: open discussion.
    • Round-robin: each member contributes in turn.

“Five Why” Technique

  • Ask “Why?” about the problem 5 sequential times.
  • Drills down to the underlying root cause (e.g., inadequate resources → poor training → system failure).
  • Stop when a fundamental, actionable cause is reached.

Fishbone (Ishikawa) Analysis

  • Visual diagram shaped like a fish; head = problem, bones = cause categories.
  • Typical categories: People, Process, Environment, Materials, Equipment, Management.
  • Helps list and group multiple contributing factors (e.g., workload, anxiety, poor study materials leading to exam failure).

Dot-Voting Prioritization

  • Participants receive dots/stickers to vote on listed causes.
  • Highest-voted causes become focus for corrective action.

Key Takeaways

  • Combine brainstorming to generate causes, Five Why or Fishbone to reach root causes, then Dot Voting to prioritize.
  • End goal: clear, agreed-upon root causes that guide targeted solutions.
