Right Patient

Right Drug

Right Dose
Verifying that the medication is being dispensed at the appropriate dose for the patient, based on factors such as age, weight, medical condition, and other medications they may be taking. Pay extra attention to high-alert drugs.
Right Route of Administration
Confirm that the route of administration matches the dosage form that is prescribed, such as oral, topical, or intravenous.
Right Time
Ensure that the medication is being administered at the appropriate time as prescribed, whether it is a one-time dose or a medication that is given on a regular schedule.
Right Documentation
Accurate documentation is essential in identifying any changes made with a prescription, such as allergies, a patient request, or for insurance billing.
Right Education
The offer for counseling by a pharmacist is required with every prescription. Ensure Medication Guides are given with every drug that requires one. Alert patient to any changes in their prescription.
