Assessment Objectives

Global Criteria

  • Explain assessment results to the client
    • Communicate findings clearly to enhance client understanding and involvement.
  • Identify and rank the problems needing resolution
    • Collaborative effort to pinpoint crucial issues to address in treatment.

Goal Setting

Global Criteria

  • Formulate agreed-upon immediate and long-term goals
    • Establish clear and achievable objectives for the client’s treatment journey.
  • Identify treatment methods and resources to be utilized
    • Select appropriate interventions tailored to the client's needs.

Treatment Plan Requirements

  • Basis of the client’s treatment plan is the assessment
    • Treatment should stem from comprehensive evaluation of the client.
  • JAHCO Requirements:
    • Treatment plan must be based on Intake information.
    • Must be formulated within 72 hours of Intake.

Individualized Treatment

  • The treatment plan should be individualized
    • Customized approaches based on specific client circumstances and goals.

Collaborative Problem-Solving

  • Client and Counsellor list the significant problems and rank them
    • Joint activity to ensure all pertinent issues are addressed and prioritized in the treatment plan.
