For patients undergoing a colonoscopy, thorough bowel preparation is required. Nursing care also encompasses mouth care, utilizing sponges and keeping the lips moist to ensure patient comfort during periods of restricted oral intake.

Celiac Disease: Pathophysiology and Clinical Manifestations

Celiac disease is a multi-system autoimmune disorder characterized by chronic inflammation of the small intestinal mucosa. This inflammatory process leads to bowel wall atrophy, malabsorption, and diarrhea. It is essential for patients to understand the frequency of food intake and the specifics of their diet. The only effective treatment for celiac disease is a strict gluten-free diet (GFD). While the damage to the intestinal mucosa can sometimes return to normal with dietary adherence, the disease carries risks for significant complications, including Non-Hodgkin lymphoma, various gastrointestinal (GI) cancers, and chronic nutritional deficiencies.

Symptoms of celiac disease include significant weight loss, anorexia, diarrhea, constipation, steatorrhea (fatty stools), vomiting, abdominal pain, and abdominal distention. Patients must be taught about the necessary vitamins and minerals required to supplement their diet. Diagnosis typically involves screening blood tests followed by an endoscopy. Management requires collaborative efforts with a dietician or nutritionist. Patients may also need to learn to administer supplements for conditions like anemia or B12B_{12} deficiency themselves.

Cholecystitis: Causes, Genetics, and Risk Factors

Cholecystitis is the inflammation of the gallbladder, which can be classified as either acute or chronic. Acute cholecystitis can occur with or without the presence of gallstones (calculi). Causes of acalculous cholecystitis (without stones) include trauma, sepsis, long-term TPN, major abdominal surgery, and hypovolemia. Chronic cholecystitis generally results from repeated episodes of cystic duct obstruction, which leads to chronic inflammation; in these cases, calculi are almost always present.

Genetic and familial tendencies play a role in the development of cholecystitis. The condition is more prevalent in women than in men and is seen more frequently in individuals living in affluent countries. Other significant risk factors include Type 22 Diabetes Mellitus, gastric bypass surgery, Crohn’s disease, rapid weight loss, sickle cell disease, obesity, and diets high in fat and cholesterol. Health promotion focuses on teaching healthy lifestyle habits, preventing obesity, and regular exercise.

Assessment and Diagnostic Findings in Cholecystitis

When assessing for cholecystitis, clinicians look for right upper quadrant abdominal pain that may radiate to the right shoulder. Other signs include anorexia, nausea, vomiting, dyspepsia (an epigastric burning sensation or heartburn), eructation (belching), and flatulence. A key physical finding is Blumberg’s sign, which is characterized by rebound tenderness during palpation. Chronic cases may present with jaundice, clay-colored stools, dark urine, and steatorrhea.

Laboratory assessments typically show an increase in the White Blood Cell (WBCWBC) count, as well as elevations in alkaline phosphatase, Aspartate Aminotransferase (ASTAST), and Lactate Dehydrogenase (LDHLDH). Serum bilirubin levels may also be increased. Diagnostic imaging includes X-rays and ultrasound. More invasive procedures like Endoscopic Retrograde Cholangiopancreatography (ERCP) or Magnetic Resonance Cholangiopancreatography (MRCP) are used to visualize the gallbladder and liver, with MRCP usually being performed prior to the more invasive ERCP.

This is often temporary as the body adjusts to the absence of the gallbladder, but it requires patient education regarding pain relief and nutrition interventions. The overall goal of care is to manage acute or persistent pain (often using opioids) and promote adequate nutrition to meet metabolic needs.

Peptic Ulcer Disease (PUD): Etiology and Complications

Peptic Ulcer Disease (PUD) occurs when the mucosal defenses of the gastrointestinal tract become impaired, leaving the epithelium unprotected from the corrosive effects of acid and pepsin. Ulcers can be duodenal, gastric, or stress-related. A significant number of ulcers are caused by Helicobacter pylori (H. pylori), which is transmitted through fecal-to-oral or oral-to-oral (via vomiting) routes. Long-term use of NSAIDs is another major cause, particularly in high-risk groups such as those managing osteoarthritis. Other risk factors include smoking, alcohol use, caffeine intake, stress, and genetics.

There are four major complications associated with PUD. First is hemorrhage, often diagnosed when a patient presents with a GI bleed; symptoms include being pale, low blood pressure (BPBP), tachycardia, weak pulses, and being at risk for shock. Second is perforation, which is a life-threatening emergency. Perforation leads to peritonitis, where the abdomen becomes hard and "board-like" with sudden, sharp, mid-epigastric pain that spreads. Third is pyloric obstruction, which causes vomiting due to gastric contents being unable to pass. Fourth is intractable disease, where the patient deals with chronic ulcers that do not respond well to standard treatments.

Diagnostics and Triple Therapy for PUD

The gold standard for diagnosing Peptic Ulcer Disease is an Esophagogastroduodenoscopy (EGD). Other diagnostic tests include testing for H. pylori, checking hemoglobin (HgbHgb) and hematocrit (HctHct) for anemia, screening for occult blood in the stool, and nuclear medicine scans. A psychosocial assessment is also important to understand how the patient copes with stress, including their use of alcohol or tobacco.

Standard medical treatment for H. pylori and ulcer healing involves a triple therapy protocol. This typically includes a Proton Pump Inhibitor (PPI) such as Protonix, and two antibiotics, such as Ciprofloxacin (Cipro) and Metronidazole (Flagyl). Nutritional interventions include avoiding spicy foods. Success of the treatment is evidenced by a decrease in WBCWBC count, reports of decreased pain, a less tender abdomen, and improved intake and output. Patients must be monitored for worsening signs such as sudden changes in blood pressure, tachycardia, or the abdomen becoming harder, which suggests perforation.

Viral Hepatitis: Types and Transmission

Hepatitis is an inflammation of the liver, most commonly caused by a virus. Liver cells (hepatocytes) can undergo necrosis but also have the ability to regenerate over time, especially during rest and sleep. There are five main types: A, B, C, D, and E. Hepatitis A is transmitted via the fecal-oral route through stagnant water, contaminated food, or undercooked shellfish. It is characterized by flu-like symptoms and is not considered a chronic condition. It can be killed by bleach or high heat. A vaccine is available, and an immunoglobulin injection can be given within 1414 days of exposure.

Hepatitis B is transmitted through blood and blood-derived body fluids via percutaneous or permucosal routes (e.g., sharing razors, toothbrushes, needle sticks). It can become chronic. High-risk groups include those who share needles, diabetic patients receiving injections, baby boomers (due to lacks in blood screening before 19921992), and infants born to infected mothers. Hepatitis C is also bloodborne (blood-to-blood) and is a leading cause of End-Stage Renal Disease (ESRD). It is almost always chronic and difficult to identify early due to a lack of symptoms. While there is no specific vaccine for Hep C, vaccination for Hep A and B is encouraged for these patients.

Clinical Manifestations and Laboratory Values in Hepatitis

The acute phase of hepatitis lasts from 11 to 44 months and is the period of maximal infectivity. Symptoms during the incubation and acute phases include malaise, anorexia, fatigue, nausea, vomiting, and abdominal pain. Physical examination may reveal hepatomegaly (enlarged liver), lymphadenopathy (enlarged lymph nodes), and splenomegaly (enlarged spleen). Patients may be icteric (showing jaundice and yellowish sclera) or anicteric. Other findings include arthralgia (joint pain), myalgia (muscle pain), clay-colored stools, dark brownish urine, dry skin, and pruritus (itching), which is often treated with Benadryl.

Key laboratory values in hepatitis show increases in ASTAST (848U/L8-48\,U/L), ALTALT (755U/L7-55\,U/L), LDHLDH (122222122-222), and bilirubin (0.11.2mg/dL0.1-1.2\,mg/dL). Ammonia (NH3NH_3) levels may rise to 1545u/dL15-45\,u/dL, which can negatively affect the level of consciousness (LOCLOC). Conversely, total protein (6.37.9g/dL6.3-7.9\,g/dL) and albumin (3.55.0g/dL3.5-5.0\,g/dL) levels typically decrease. A prolonged prothrombin time (PTPT) of 9.5-13.8\,\text{seconds} indicates a high bleeding risk.

Nursing Interventions and Prevention of Hepatitis

Nursing care for hepatitis focuses on addressing weight loss and extreme fatigue. Patients should be provided with rest periods and encouraged to stay independent where possible, although assistance may be needed for ADLs. Nutritional support includes a diet high in carbohydrates, with low fat and protein introduced only after nausea subsides. Small, frequent meals and vitamin supplements are beneficial. When scheduling appointments, nurses should identify when the patient has the most energy.

Prevention is centered on sanitation and personal hygiene. Key strategies include washing hands after using the toilet and before eating, drinking only purified or bottled water (especially when traveling in underdeveloped countries), and avoiding ice or raw vegetables washed in tap water. Individuals should never share needles, razors, nail clippers, or toothbrushes. Safe practices such as using condoms or abstaining from sexual activity are recommended. Those infected with hepatitis are permanently disqualified from donating blood, organs, or tissues.
