Inflammation Overview
Definition of Inflammation: Inflammation is the body's second line of defense after physical barriers such as the skin. Its purpose is to respond to breaches in the body's barrier, allowing for healing and protection against pathogens.
Example: If skin is damaged (e.g., from a bee sting), inflammation activates to assist in healing.
Types of Inflammatory Responses:
Local Response: Occurs in a specific area of the body.
Example: Bee sting results in localized swelling, redness, heat, and loss of function in the thumb.
Systemic Response: Involves the entire body.
Example: Reaction to a severe allergen or infection can result in fever (temperature increase), tachycardia (elevated heart rate), tachypnea (elevated respiratory rate), leukocytosis (white blood cell count > 10,000), and general malaise/fatigue.
Local vs. Systemic Inflammatory Responses
Local Response: Characterized by symptoms like swelling and pain confined to the area of injury. No systemic symptoms such as increased heart rate.
Systemic Response: Indicators include elevated heart rate, elevated respiratory rate, and fever, indicating the body's extensive response to a threat.
Vascular Response to Injury
Initial Vasoconstriction: To prevent excessive bleeding after tissue injury.
Histamine Release: After vasoconstriction, injured cells release histamine, leading to vasodilation (increase in blood flow) and increased permeability of capillaries, allowing fluid, immune cells, and nutrients to reach the damaged area.
Exudate Formation: Fluid rich in proteins, leukocytes, and cellular debris accumulates at the site of injury as exudate, aiding in cleaning the area.
Cellular Response to Injury
Phagocytosis: Phagocytes (a type of white blood cell) engulf pathogens and debris to clear the injury site.
Healing Process: Tissues may regenerate (e.g., liver cells) or form scar tissue if they are unable to regenerate (e.g., skin).
Is Inflammation Beneficial or Detrimental?
Positive Aspect: Necessary for healing and protection against pathogens.
Negative Aspect: Prolonged or repeated inflammation can lead to tissue damage and chronic health issues.
Distinction Between Inflammation and Infection
Inflammation can occur without infection (e.g., injury) and vice versa.
Example: Gout causes inflammation without infection due to uric acid crystal deposits in joints.
Gout: An Example of Inflammatory Response
Pathophysiology: Caused by the accumulation of uric acid, leading to crystal formation in joints, triggering an inflammatory response.
Symptoms of Gout Attack: Redness, swelling, intense pain, particularly in the big toe.
Acute vs. Chronic Condition: Acute pain episodes occur due to crystal accumulation, while gout is a chronic condition.
Risk Factors:
High purine diet (meat, organ meats, seafood, alcohol).
Obesity.
Kidney function impairment.
Male gender.
Family history of gout.
Gout Management
Acute Treatment:
NSAIDs: Nonsteroidal anti-inflammatory drugs (e.g., Ibuprofen, up to 1200 mg).
Colchicine: Another anti-inflammatory treatment for gout.
Corticosteroids: For severe cases but used infrequently.
Preventative Treatment:
Allopurinol: Reduces uric acid production.
Other medications include probenecid and Krystexxa.
Lifestyle Modifications:
Limit alcohol and high-purine foods (meats, seafood).
Encourage hydration for kidney function.
Avoid diuretics and aspirin (due to effects on uric acid levels).
GERD: A Different Inflammatory Response
Definition: Gastroesophageal reflux disease (GERD) is caused by stomach acid flowing back into the esophagus, damaging tissue and triggering the inflammatory response.
Pathophysiology: Repeated exposure to acidic contents inflames the esophagus, with symptoms such as heartburn and damage to the esophageal lining.
Risk Factors:
Obesity.
Hiatal hernias.
Pregnancy (increased abdominal pressure).
Connective tissue disorders affecting esophageal sphincter function.
Smoking.
Delayed stomach emptying.
Consumption of irritating foods/drinks (alcohol, fried foods).
Diagnosis: Symptoms history, possible endoscopy or X-rays.
GERD Management
Medications:
Antacids: Reduce acidity, e.g., Tums, Mylanta.
Histamine Blockers: Reduce acid production (e.g., famotidine, Pepcid).
Proton Pump Inhibitors: Prescription strength for severe cases (e.g., omeprazole).
Lifestyle Modifications:
Maintain a healthy weight and quit smoking.
Elevate the head during sleep.
Avoid eating large meals and lying down post-meal.
Identify and avoid trigger foods (e.g., caffeine, chocolate).
