Overview of the Spinal Cord

  • The spinal cord is a key structure in the central nervous system (CNS) responsible for receiving and sending information throughout the body.

  • It plays a crucial role in reflexes and nerve communication.

  • It connects to both somatic (skeletal) muscles and visceral (internal organs) systems.

  • Spinal reflexes operate independently from the brain, responding directly to stimuli.

Structure of the Spinal Cord

General Structure

  • The spinal cord resembles a butterfly shape in cross-section with an internal gray matter area and surrounding white matter.

  • It is divided into two symmetrical halves, each containing grooves and fissures to denote anterior and posterior sides.

  • The anterior median fissure is a deeper groove on the front side, while the posterior median sulcus is a shallower indentation on the back side.

Enlargements

  • The spinal cord has two notable enlargements:

    • Cervical enlargement: located where nerves supplying the upper limbs emerge, resulting in a thicker appearance.

    • Lumbar enlargement: corresponding to the nerves supplying the lower limbs and pelvis.

Terminology

  • Conus medullaris: the tapered, cone-shaped end of the spinal cord, typically around the junction of lumbar vertebrae L1 and L2.

  • Filum terminale: a single filament extending from the conus medullaris, helping to stabilize the spinal cord.

  • Cauda equina: a bundle of spinal nerves and their roots located below the conus medullaris, resembling a horse's tail (the term "cauda" means tail).

Spinal Segments and Nerves

  • There are 31 pairs of spinal segments, which correspond to pairs of spinal nerves:

    • Cervical nerves: 8 pairs (C1-C8)

    • Thoracic nerves: 12 pairs (T1-T12)

    • Lumbar nerves: 5 pairs (L1-L5)

    • Sacral nerves: 5 pairs (S1-S5)

    • Coccygeal nerve: 1 pair (Co1)

Spinal Nerves and Their Functions

Structure of Spinal Nerves

  • Each spinal nerve is composed of:

    • Dorsal root: carries sensory information to the CNS, containing sensory neuron axons and the dorsal root ganglion (neuron cell bodies).

    • Ventral root: carries motor signals out from the CNS, containing axons of motor neurons.

  • When combined, these roots form a mixed spinal nerve that contains both sensory and motor signals.

Dorsal Root Ganglion

  • The dorsal root ganglion contains the cell bodies of unipolar sensory neurons, leading to a distinct bump on the dorsal root.

Functional Aspects of the Spinal Cord

Reflex Arcs

  • Reflex arcs involve a direct pathway from sensory input to motor output and are contained within the spinal cord, allowing for quick responses without direct involvement of the brain.

  • Reflexes can be voluntary (involving brain response) or spinal reflexes (involving just the spinal cord).

Gray and White Matter

  • Gray matter: located in the center, contains unmyelinated axons, cell bodies, and synapses; organized into anterior, lateral, and posterior horns.

  • White matter: surrounds gray matter, contains myelinated axons and is divided into columns for ascending (sensory) and descending (motor) pathways.

Meninges of the Spinal Cord

  • Three protective layers surround the spinal cord:

    • Pia mater: the innermost layer that hugs the spinal cord.

    • Arachnoid mater: the middle layer, with the subarachnoid space that contains cerebrospinal fluid (CSF).

    • Dura mater: the outermost and toughest layer.

Clinical Relevance

  • Meningitis can occur if pathogens invade the meninges, emphasizing the importance of vaccinations.

  • A lumbar puncture (spinal tap) can be performed to diagnose meningitis by analyzing CSF for bacteria or other abnormalities.
