Pathophysiology of Cardiovascular Disease

  • Cardiovascular disease development begins at the level of the endothelium of the vasculature. In a healthy state, the endothelium forms a nice, continuous connection, but over time, damage can create gaps in this lining.

  • Specific ailments and factors that lead to endothelial damage include:

    • Hypertension (high blood pressure).
    • Smoking.
    • Hyperglycemia (even prior to full hyperglycemia, insulin resistance plays a role).
    • Hyperhomocysteinemia (elevated levels of homocysteine).
    • Chronic inflammation, even if it is low-level inflammation.
  • The progression of atherosclerosis involves several distinct stages:

    • LDL Deposition: Low-Density Lipoprotein (LDL) cholesterol in circulation becomes deposited beneath the endothelial lining within the tunica intima.
    • Role of Small, Dense LDL: Deposition occurs at a higher rate if there is a higher percentage of small, dense LDL particles. These particles become embedded in the tunica intima more effectively. Prolonged hyperglycemia is a factor that contributes to a higher proportion of these small, dense particles.
    • Immune Response and Oxidation: White blood cells respond to the presence of LDL in the tunica intima. These white blood cells enter the area and release free radicals. During this time, the deposited LDL becomes oxidized, which is when the damage becomes significantly more severe.
    • Feedback Loop: An inflammatory response is triggered as white blood cells enter, creating a feedback loop that worsens the condition.
    • Fatty Streak Formation: A lipid plaque forms from a combination of dead white blood cells and smooth muscle cells that migrate from external layers into the tunica intima. This formation is known as a "fatty streak."
    • Plaque Progression: While the fatty streak itself is not immediately dangerous, it eventually becomes fibrous. This fibrous buildup perpetuates inflammation and creates plaque instability. This instability can lead to a plaque rupture, which causes the major health concerns associated with cardiovascular disease.

The Lipid Debate and the Nuanced Approach

  • There are two primary perspectives (or "camps") regarding the cause of atherosclerosis:

    • Lipid-Centric Camp: Focuses heavily on the lipid panel and the role of LDL in perpetuating atherosclerosis.
    • Endothelial-Centric Camp: Argues that endothelial cell damage is the primary driver, suggesting that if damage is prevented, the lipid panel results matter less.
  • The instructor emphasizes a nuanced approach, noting that focusing solely on lipids ignores preceding factors that cause endothelial damage. However, ignoring the lipid panel entirely is considered irresponsible.

  • Case Study: H LP (Hyperlipidemia):

    • Evidence from individuals with H LP shows that high LDL and high total cholesterol are significant risk factors.
    • These individuals, particularly those with severe elevations, are at high risk for very premature atherosclerosis and myocardial infarction (MIMI).
    • Crucially, this increased risk exists even in the absence of other factors like hypertension, smoking, or hyperglycemia, demonstrating that lipid levels are an independent risk factor.

Major Risk Factors for Cardiovascular Disease

According to the American Heart Association (AHAAHA), major risk factors are those highly correlated with atherosclerosis and cardiovascular disease:

  • Smoking: Directly perpetuates endothelial damage.
  • High Blood Pressure (Hypertension): Damages endothelial cells, leading to excess cardiac risk.
  • Lipid Panel Findings: High LDL cholesterol and low HDL cholesterol are considered major risk factors due to their high correlation with atherosclerosis in existing literature.
  • Diabetes: This condition worsens endothelial damage, increases the production of small, dense LDL particles, and increases the oxidation of LDL cholesterol.
  • Age:
    • Males: 45≥ 45 years old.
    • Females: 55≥ 55 years old. This difference is attributed to a roughly ten-year lag in females due to the onset of menopause; after menopause, the risk between males and females equalizes.
  • Family History: This refers to first-degree relatives (father, mother, sister, or brother).

    • Female relative: Experienced a cardiac event under the age of 6565.

Risk Enhancing Factors

These factors have a recognized association with risk, though they may not be as highly correlated as the major factors:

  • Obesity: Associated with increased cardiovascular risk.
  • Sedentary Lifestyle: Lack of physical activity.
  • Diet: Specifically the "Standard American Diet," which is characterized as being high in saturated fats, high in simple carbohydrates, and low in fiber.
  • Alcohol Consumption: While there is debate regarding moderate consumption, high consumption is a risk factor, and minimal consumption is generally recommended.
  • Stress and Psychosocial Factors: Includes chronic stress, anxiety, depression, and social isolation.
  • Health Disparities: This includes socioeconomic status, education, access to healthcare, and access to quality foods. Individuals living in "food deserts" are at a higher risk for atherosclerosis.
  • Elevated Triglycerides: Considered a risk-enhancing factor.
  • Metabolic Syndrome: A cluster of conditions that significantly impact cardiovascular risk.

Additional Lab Indicators and Medical Conditions

Other lab assessments used to determine cardiovascular risk include:

  • Lp(a)Lp(a) (Lipoprotein little a): An additional indicator of risk mentioned in previous discussions.
  • Homocysteine: High levels lead to increased endothelial damage.
  • Growth Factors (Fibrinogen): Elevated levels of fibrinogen are linked to increased risk.
  • CRP (C-Reactive Protein): A marker for inflammation; elevated levels increase the damage occurring at the level of the endothelium.
  • Sleep Apnea: There is a strong positive correlation between sleep apnea and cardiovascular disease risk.
