Neurovascular Bundle

  • Definition: The neurovascular bundle consists of nerves, arteries, and veins.

  • Arrangement: Close to muscles and supportive structures, meant to nourish and support those muscles.

Common Injuries Related to Repetitive Use and Trauma

  • Carpal Tunnel Syndrome

    • Definition: Entrapment of the median nerve at the wrist.

    • Anatomy:

    • The wrist contains eight carpal bones shaped like a 'C', which are arranged to form a tunnel through which the flexor digitorum vessels, tendons, and the median nerve pass.

    • The transverse collateral ligament (flexor retinaculum) maintains the structure of the carpal tunnel.

    • Causes:

    • Repetitive use or trauma leading to inflammation and swelling in the wrist area, which results in nerve compression.

    • Occupations at risk include:

    • Typists, keyboard data analysts, pianists, and construction workers (e.g., jackhammer users).

    • Symptoms:

    • Pins and needles sensation (paresthesia) or numbness at the radial side of the fourth digit (fingers).

    • Pain can be reproduced by palpating the area or performing Phalen's test.

    • Diagnosis: Primarily based on clinical history and physical examination.

    • Treatment options:

    • NSAIDs, splinting (especially at night), steroid injections (not into the tendon), and surgical intervention (cutting the transverse ligament to relieve pressure).

  • Thoracic Outlet Syndrome (TOS)

    • Definition: Compression of the neurovascular bundle in the neck, usually due to anatomical abnormalities such as an anomalous first rib or tight scalene muscles.

    • Types:

    • Neurogenic: Most common, involves compression of the brachial plexus.

    • Vascular: Involves compression of the subclavian vein or artery.

    • Symptoms:

    • Patients may experience pain, weakness, or numbness that can radiate into the upper extremities.

    • Diagnosis: Medical imaging like X-rays to check for cervical rib anomalies or physical examination tests (e.g., testing for diminished pulse or pain reproduction).

    • Initial Diagnostic Approach: Use of plain radiographs and potentially CT or MRI for confirmation

    • Treatment options: Physical therapy, medications, or surgery, especially for anatomical defects.

Costochondritis vs. TEK Syndrome

  • Costochondritis

    • Definition: Inflammation at the junction of the ribs and the sternum.

    • Commonly affects ribs two through five, with rib two being the most frequently affected.

    • Symptoms:

    • Point tenderness and chest pain reproducible on palpation.

    • Treatment: Usually self-limiting; NSAIDs can help manage pain.

  • TEK Syndrome

    • Similar to costochondritis but includes swelling and edema at the point tenderness.

    • Severer presentation than costochondritis but does not differ significantly in terms of treatment.

Complex Regional Pain Syndrome (CRPS)

  • Definition: A condition previously known as reflex sympathetic dystrophy (RSD) or algodystrophy.

  • Prevalence: Affects about 2% to 17% of patients post-injury.

  • Demographics: More common in females; can manifest in upper or lower extremities depending on age group.

  • Symptoms:

    • Characterized by significant pain, regional edema, discoloration of the skin, hair loss, and muscle atrophy.

  • Triggers: Often follows a traumatic event such as surgery or fracture.

  • Diagnostic Imaging: Bone scans are preferred for this condition, allowing for triple phase imaging.

    • Detects increased uptake of technetium in affected areas, indicative of pathology.

  • Treatment options: include NSAIDs, antidepressants, anticonvulsants, and in severe cases, surgical options such as sympathectomy to relieve symptoms.
