Neuro Part 1

Assessment of the Nervous System

  • Dendrites: Projections of a neuron that receive signals from other neurons. They play a crucial role in the transmission of information within the nervous system.

  • Cell body (soma): The central part of a neuron that contains the nucleus and is responsible for maintaining the life of the cell and processing information.

  • Axon: A long, slender projection of a neuron that transmits signals away from the cell body to other neurons or muscles.

  • Node of Ranvier: Gaps in the myelin sheath of axons that facilitate rapid conduction of electrical signals through saltatory conduction.


CNS: Structure and Function

  • Central Nervous System (CNS): Comprises the brain and spinal cord.

    • Brain: Divided into several major regions:

    • Forebrain: Responsible for higher cognitive functions, emotions, and sensory processing.

    • Cerebellum: Coordinates muscular movements and balance.

    • Brainstem: Controls fundamental life functions such as heart rate and breathing.

    • Spinal Cord: Transmits signals between the brain and the rest of the body and processes reflex actions.


Peripheral Nervous System: Structure and Function

  • Peripheral Nervous System (PNS): Connects the CNS to limbs and organs. It is divided into:

    • Spinal Nerves: Mixed nerves that carry motor, sensory, and autonomic signals between the spinal cord and the body.

    • Cranial Nerves: Nerves that directly emerge from the brain and serve various functions including sensory and motor functions in the head and neck.

    • Autonomic Nervous System: A component of the PNS that regulates involuntary bodily functions.

    • Sympathetic Nervous System: Prepares the body for stressful or emergency situations, triggering the “fight or flight” response.

    • Parasympathetic Nervous System: Contributes to the “rest and digest” functions, promoting activities that occur when the body is at rest.


Neurologic Changes Associated with Aging

  • Changes often impact several areas:

    • Mobility: Declines in motor function can affect overall mobility.

    • Sensory Perception: Changes in taste, smell, sight, and hearing can occur.

    • Cognition: May experience declines in cognitive function.

    • Motor Changes: Can include problems with coordination, balance, and strength.

    • Functional Status Assessment: It is important to evaluate the functional capabilities of aging individuals.


Health Promotion and Maintenance

  • Prevention: Focus on avoiding risky behaviors and practicing a healthy lifestyle.

    • Sleep: Ensure an adequate amount of sleep for recovery and maintenance of health.

    • Proper Nutrition: Follow a balanced diet to support overall health and neurologic function.


Assessment: Noticing and Interpreting

  • Comprehensive evaluation approaches include:

    • Patient History: Gathering a detailed account of the patient’s health history.

    • Physical Assessment: Conducting a hands-on examination to assess physical health.

    • Complete Neurological Assessment: Involves detailed examination of various aspects:

    • Mental Status: Evaluating cognitive function and mood.

    • Cranial Nerves: Assessing function of the cranial nerves.

    • Motor Function: Testing strength and coordination.

    • Reflex Activity: Checking for both deep and superficial reflexes.

    • Sensory Function: Assessing the ability to perceive sensory stimuli.

    • Cerebellar Function: Evaluating balance and coordination.


Posturing

  • Assessment of posturing in patients can provide insights into neurological function or distress. Posturing responses may indicate severe brain injury or dysfunction.


Glasgow Coma Scale

  • Glasgow Coma Scale: A scale used to assess a patient's level of consciousness. The highest possible score is 15.

    • Eye Opening:

    • Spontaneous

    • To sound

    • To pain

    • Never

    • Motor Response:

    • Obeys commands

    • Localizes pain

    • Normal flexion (withdrawal)

    • Abnormal flexion

    • Extension

    • Verbal Response:

Psychosocial Assessment

  • Patient responses to their condition can differ significantly based on whether the condition is acute or chronic.

  • Depression Screening Tool: Important to incorporate tools for assessing depression and other mental health issues in patients.


Diagnostic Assessment

  • Diagnostic assessments may include:

    • Laboratory Assessment: Various tests to assess biochemical markers in the body.

    • Imaging Assessment:

    • Plain X-rays: Basic imaging technique to assess bone structures.

    • Cerebral Angiography: Imaging of the blood vessels in the brain.

    • Computed Tomography (CT) Scan: Detailed imaging that provides cross-sectional views of the brain.

    • Magnetic Resonance Imaging (MRI): Offers detailed images using magnetic fields and radio waves.

    • Positron Emission Tomography (PET): Assesses metabolic activity and function of organs.

    • Single-Photon Emission CT (SPECT): A nuclear imaging test measuring blood flow to tissues.

    • Magnetoencephalography (MEG): Measures magnetic fields produced by electrical activity in the brain.


Electromyography (EMG)

  • Used to identify disorders related to nerves and muscles, as well as spinal cord diseases. This test records electrical activity of muscles to assess their health.


Electroencephalography (EEG)

  • Electroencephalography (EEG): A test that graphically records the electrical activity of the cerebral hemispheres. It is used to diagnose conditions such as epilepsy, sleep disorders, and other brain disorders.


Lumbar Puncture

  • Procedure conducted typically at the L4-L5 level to extract cerebrospinal fluid for testing or to relieve pressure in the CNS.


Other Studies

  • Transcranial Doppler Ultrasonography: A technique used to assess blood flow in the brain by measuring the velocity of blood flow through the brain's blood vessels.
