Chapter 26: Introduction to the Digestive System

1. Organs of the Gastrointestinal (GI) Tract

  • The GI tract is also known as the digestive tract or alimentary canal.

  • It is a continuous tube consisting of the following organs:
      - Oral Cavity (Mouth)
      - Pharynx (Throat)
      - Esophagus
      - Small Intestine
      - Large Intestine
      - Anus (end)

  • Key Features:
      - The GI tract is internally lined with the mucous membrane.
      - Lumen: The empty space inside the tube where ingested food is broken down into smaller components that can be absorbed.
      - Mucous Membrane Function: Digestion occurs within the lumen, and absorption happens when nutrients cross the mucosa into the body.

2. Accessory Digestive Organs and Structures

  • These organs assist in the breakdown of food and develop from outgrowths of the GI tract itself. They include:
      - Oral Cavity: Includes teeth and tongue, which participate in chewing and swallowing.
      - Salivary Glands: Secrete saliva to aid in digestion. Done my the pancreas and small intestine
      - Liver: Produces bile, which helps emulsify fats.
      - Gallbladder: Concentrates and stores liver secretions (bile).
      - Pancreas: Produces digestive enzymes that help in food breakdown.

3. Six General Functions of the Digestive System

  • Ingestion: The introduction of food and liquids into the oral cavity, marking the first step in digestion and nutrient absorption.

  • Motility: Involves mixing and moving materials through the GI tract via voluntary and involuntary muscular contractions.

  • Secretion: Release of digestive juices, including enzymes, bile acids, and mucus, into the GI tract to facilitate digestion.

  • Digestion: The breakdown of ingested food into smaller components for absorption, through:
      - Mechanical Digestion: Physical breakdown of food without changing its chemical structure.
      - Chemical Digestion: The activity of specific enzymes that alter molecules to facilitate absorption.

  • Absorption: The process where nutrients, vitamins, and water pass from the lumen of the GI tract across the mucosa into the bloodstream or lymphatic system, also performed by bacteria in the small intestine.

  • Elimination: The expulsion of undigested components and waste products through the anal canal.

4. Four Tunics (Layers) of the Gastrointestinal Wall

  • Mucosa: The inner lining of the GI tract, composed of three main components:
      - Epithelium: Direct contact with the lumen, responsible for secretion and absorption.
        - Stomach, Small Intestine, and Large Intestine: Lined with simple columnar epithelium.
        - Esophagus: Lined with non-keratinized stratified squamous epithelium.
      - Lamina Propria: Underlying layer that supports the epithelium.
      - Muscularis Mucosa: A thin layer of smooth muscle facilitating movement of the mucosa.

  • Submucosa: Composed of areolar and dense irregular connective tissue containing:
      - Blood vessels, lymph vessels, nerves, and glands.
      - Submucosal Nerve Plexus: Autonomic neurons innervating smooth muscles and glands in the mucosa and submucosa.
      - Mucosa-associated lymphoid tissue (MALT): Provides defense against pathogens, preventing the passage of harmful agents from the GI tract to the body.

  • Muscularis Layer: Consists of smooth muscle with two main components:
      - Inner Circular Layer: Smooth muscle cells arranged circumferentially to regulate lumen diameter and form sphincters.
      - Outer Longitudinal Layer: Cells oriented lengthwise within the wall, facilitating propulsion.
      - Myenteric Nerve Plexus: Fine branches of nerves coordinating peristalsis and muscular contractions.

  • Adventitia or Serosa: The outermost layer which can either be:
      - Adventitia: Composed of areolar connective tissue, anchoring organs to surrounding structures.
      - Serosa: Adventitia plus visceral peritoneum, reducing friction for intraperitoneal organs.

5. Process of Absorption

  • Absorption: Nutrients pass from the lumen of the GI tract to the mucosal epithelium and then enter the bloodstream or lymphatic system within the submucosa.

6. Comparison of Muscularis Mucosa and Muscularis Tunic

  • Muscularis Mucosa: Produces localized movements of the mucosa to enhance absorption.

  • Muscularis External: Responsible for larger contractions that generate peristalsis to move food through the digestive tract.

7. Enteric Nervous System and Autonomic Nervous System Regulation

  • Enteric Nervous System (ENS): A complex network of nerves that regulates reflexes and activities within the GI tract.

  • Autonomic Nervous System (ANS): Influences the digestive process by modulating the activity of the ENS, promoting the secretion of digestive enzymes and coordination of muscular contractions.
