Urinary Tract Infections (UTIs)

  • Common Infections

    • Very prevalent; urines serve as a good growth medium.
    • Most UTIs are caused by Escherichia coli and majority are ascending (begin in the urethra and move upward).
    • Can lead to life-threatening complications like urosepsis.
  • Types of UTIs

    • Lower UTIs
    • Cystitis: inflammation of the bladder.
    • Urethritis: inflammation of the urethra.
    • Upper UTIs
    • Pyelonephritis: infection spreads to the kidneys, resulting in severe complications.
  • Risk Factors

    • Women are more susceptible due to shorter urethra and proximity to the anus.
    • Older men may suffer from prostatic hypertrophy leading to urine retention.
    • Congenital abnormalities and bladder instrumentation can increase risk.
  • Symptoms of Lower UTIs

    • Painful urination, frequent urges, bladder pain in women, prostate pain in men.
    • Foul-smelling or cloudy urine.
  • Symptoms of Upper UTIs (Pyelonephritis)

    • Pain is present in lower back/flank area, systemic signs like fevers.
    • Dull aching pain, presence of leukocytes in urine.
  • Management

    • Supportive care including pain management, IV fluids if sepsis is suspected.
    • Transport to appropriate facilities for evaluation.

Urolithiasis (Kidney Stones)

  • Description

    • Also known as calculi; can occur anywhere in urinary tract.
    • Form due to excessive solutes in urine, insufficient fluid intake, or infections.
  • Types of Stones

    • Calcium salts, uric acid, struvite, cysteine.
    • Formation associated with high calcium, alkaline urine, or hyperuricemia.
  • Symptoms

    • Often asymptomatic until obstruction occurs; may cause flank pain, renal colic.
    • Nausea, vomiting, and signs of UTI may occur if obstruction persists.
  • Management

    • Pain relief is critical; may employ IV fluids and transport for further care.

Hydronephrosis

  • Definition

    • Accumulation of urine within the kidney due to obstruction; often asymptomatic.
  • Causes

    • Complications from calculi, tumors, prostatic enlargement, developmental abnormalities.

Renal Failure

  • Types

    • Acute Kidney Injury (AKI)
    • Sudden decline in kidney function; reversible if treated promptly. High mortality if untreated.
    • Chronic Kidney Disease (CKD)
    • Progressive loss of kidney function seen in various populations; defined stages based on GFR (Glomerular Filtration Rate).
  • Acute Kidney Injury

    • Can be prerenal, intrinsic, or postrenal in origin.
    • Prerenal involves decreased blood flow, intrinsic involves damage to kidney structures, postrenal involves urine outflow obstruction.
  • Management of AKI

    • Supportive care for ABCs (airway, breathing, circulation). May require rapid transport to prevent complications.
  • Chronic Kidney Disease Stages

    • Decreased Renal Reserve: Symptoms are absent, slight GFR reduction.
    • Renal Insufficiency: Noticeable GFR decrease, high nitrogen waste retention.
    • End-Stage Renal Failure: Very low GFR, requires dialysis or transplant.

Dialysis

  • Types

    • Hemodialysis: Blood filtered through a machine; requires access points like arteriovenous fistula.
    • Peritoneal Dialysis: Uses the peritoneum as a semipermeable membrane for waste extraction.
  • Risks & Complications

    • Common complications include hypertension, fluid overload, infections, and anemia during treatment.
    • Emergencies include symptomatic hypotension, hyperkalemia, air embolism, and disequilibrium syndrome.
  • Emergency Management

    • For emergencies like hypotensive shock or air embolism, immediate transport and supportive measures are crucial.
    • Monitoring for ECG changes in hyperkalemia is essential.
