Introduction to Cranial Nerves
Cranial nerves are mixed nerves belonging to the peripheral nervous system.
There are 12 pairs of cranial nerves, designated by Roman numerals based on their emergence from the brainstem.
Emergence from the Central Nervous System (CNS)
CN I and II: Extensions of the brain.
CN III and IV: Arise from the midbrain.
CN V: Arises from the pons.
CN VI, VII, and VIII: Emerge from the ponto-medullary junction (between pons and medulla).
CN IX, X, XI, and XII: Emerge from the medulla oblongata.
Cranial Nerve Anatomy
All cranial nerves arise from specific nuclei in the brainstem.
These nuclei contain collections of neuronal cell bodies projecting their axons, carrying specific types of fibers.
Cranial Nerve Functional Categories
General Somatic Afferents (GSA): Somatic sensation from skin and skeletal muscles.
General Visceral Afferents (GVA): Sensation from viscera, blood vessels, baroreceptors, and chemoreceptors.
General Visceral Efferents (GVE): Parasympathetic fibers to smooth muscles and glands.
General Somatic Efferents (GSE): Efferent fibers to skeletal muscles not derived from pharyngeal arches.
Special Somatic Afferents (SSA): Visual, auditory, and vestibular stimuli.
Special Visceral Afferents (SVA): Taste and olfaction (smell).
Special Visceral Efferents (SVE): Efferent fibers to skeletal muscles derived from branchial/pharyngeal arches (CNs V, VII, IX, X, XI).
Cranial Nerve I - Olfactory Nerve
Origin: Olfactory bulb
Entry: Cribriform plate
Function: Smell (SVA)
Pathway:
20-30 olfactory cell axons from the olfactory epithelium ascend through the cribriform plate.
Axons synapse on olfactory bulbs.
Olfactory tracts connect bulbs to processing centers in the brain.
Olfactory Epithelium:
Contains olfactory receptor cells.
Projects axons through the cribriform plate to synapse with mitral cells in the olfactory bulb.
Cranial Nerve II - Optic Nerve
Origin: Retina
Entry: Optic canal
Function: Vision (SSA)
Pathway:
Visual input enters through the pupil, hitting the retina.
Signals synapse at the retina and leave as optic nerve via optic canal.
Decussation of fibers occurs at the optic chiasm.
Fibers then continue as optic tracts to the visual cortex in the occipital lobe.
Decussation:
Temporal fibers carry nasal visual signals (do not cross).
Nasal fibers carry lateral signals (cross).
Clinical Relevance: Pituitary gland adenoma can compress nasal fibers, leading to visual field defects (bitemporal hemianopsia).
Cranial Nerve III - Oculomotor Nerve
Origin: Midbrain
Entry: Superior orbital fissure
Function: Eye movement, pupil constriction (GSE and GVE)
Pathway:
Leaves midbrain anteriorly, piercing dura mater to enter the cavernous sinus.
Divides into superior and inferior divisions.
Both pass through the superior orbital fissure to enter the orbit.
Divisions:
Superior Division: Innervates superior rectus and levator palpebrae superioris muscle.
Inferior Division: Innervates medial rectus, inferior rectus, and inferior oblique muscles; carries parasympathetic fibers to ciliary ganglion.
Parasympathetic Function: Sphincter pupillae contraction (pupil size reduction) and lens accommodation through ciliary body contraction.
Cranial Nerve IV - Trochlear Nerve
Origin: Dorsal midbrain
Entry: Superior orbital fissure
Function: Eye movement (superior oblique muscle; GSE)
Longest intracranial pathway, exiting from the posterior aspect of brainstem.
Pathway:
Leaves midbrain posteriorly.
Passes through cavernous sinus and enters the superior orbital fissure above common tendinous ring.
Cranial Nerve VI - Abducens Nerve
Origin: Ponto-medullary junction
Entry: Superior orbital fissure
Function: Eye movement (lateral rectus muscle; GSE)
Pathway:
Leaves the brainstem from ponto-medullary junction.
Passes through cavernous sinus inferior to internal carotid artery.
Enters superior orbital fissure and innervates the lateral rectus muscle.
Clinical Note: Most vulnerable during raised intracranial pressure, leading to inward eye turn due to damage.
Cranial Nerve V - Trigeminal Nerve
Origin: Pons
Divisions:
Ophthalmic (V1): Sensory.
Maxillary (V2): Sensory.
Mandibular (V3): Mixed (sensory and motor).
Entry:
V1: Superior orbital fissure
V2: Foramen rotundum
V3: Foramen ovale
Functions:
Provides sensation to the face and motor function for mastication.
Cranial Nerve V1 - Ophthalmic Division
Pathway:
Enters cavernous sinus.
Divides into nasociliary, lacrimal, and frontal branches at the orbital fissure.
Branches:
Frontal Nerve: Further divides into supratrochlear and supraorbital nerves for forehead and upper eyelid sensation.
Nasociliary Nerve: Involved in the sensory supply for the cornea and sclera.
Lacrimal Nerve: Supplies the lacrimal gland.
Cranial Nerve V2 - Maxillary Division
Pathway:
Enters lateral wall of cavernous sinus.
Passes through foramen rotundum to the pterygopalatine fossa.
Main Branches:
Infraorbital Nerve: Sensation for skin of midface.
Posterior Superior Alveolar Nerve: Sensation from maxillary molars and mucosa.
Zygomatic Nerve: Sensation from skin over zygomatic bone.
Pterygopalatine Fossa: Important neurovascular area with the maxillary nerve, pterygopalatine ganglion, and maxillary artery.
Cranial Nerve V3 - Mandibular Division
Pathway:
Enters infratemporal fossa via foramen ovale.
Splits into anterior and posterior trunks.
Branches:
Anterior Trunk: Supplies muscles of mastication, including masseter and temporalis.
Posterior Trunk: Includes the auriculotemporal, lingual, and inferior alveolar nerves.
Chorda Tympani: A branch providing taste fibers from the anterior two-thirds of the tongue, joining lingual nerve; carries pre-ganglionic parasympathetic fibers to the submandibular ganglion.
Summary of Dermatomes of CN V
V1 - Ophthalmic: Sensory areas include forehead, upper half of scalp, upper eyelid, upper nasal structures.
V2 - Maxillary: Sensory areas cover skin over cheeks, anterior midface, upper jaw, nasal cavity.
V3 - Mandibular: Sensory areas encompass lower jaw/chin, skin anterior to ear, anterior 2/3 of the tongue, lower teeth.
Motor Functions: V3 provides motor functions for muscles of mastication, mylohyoid, anterior belly of digastric, and tensors.
