Anatomy of the Skull

The human skull is a complex structure that consists of 22 bones interconnected primarily through sutures, which are immobile fibrous joints (except for the mandible). The skull is divided into two major sections: the cranium, which houses the brain, and the facial skeleton, which includes bones such as the maxilla, mandible, and nasal bones.

Components of the Skull
  1. Cranium: This is often referred to as the neurocranium and includes:

    • Calvaria: The upper dome of the skull, comprising the frontal and parietal bones.

    • Base: The base of the cranium, including temporal and occipital bones.

    • Parts of the cranial fossa: These include the anterior, middle, and posterior cranial fossa, formed by various bones like the frontal, sphenoid, and occipital.

  2. Facial Skeleton (Viscerocranium): This includes several key bones:

    • Maxilla: Forms the upper jaw and supports the upper teeth.

    • Mandible: The lower jaw, mobile and articulates with the temporal bone at the temporomandibular joint (TMJ).

    • Zygomatic Bone: Forms the cheekbone and lateral part of the orbit.

    • Ethmoid Bone: Contributes to the nasal cavity and orbits, containing the cribriform plate and olfactory foramina for the olfactory nerves.

    • Lacrimal Bone: Located in the inner corner of the orbit, it helps form the canal for the nasolacrimal duct.

    • Nasal Bones: Two small bones that form the bridge of the nose.

    • Palatine Bones: L-shaped bones that form part of the hard palate and the floor of the nasal cavity.

Sutures and Connections

The skull bones are interconnected by sutures, which ensure stability and allow for growth during early development:

  • Coronal Suture: Between the frontal and parietal bones.

  • Sagittal Suture: Between the two parietal bones.

  • Lambdoid Suture: Between the occipital and parietal bones.

  • Squamous Suture: Between the temporal and parietal bones.

  • Occipitomastoid Suture: Between the occipital bone and the mastoid part of the temporal bone.

Cranial Fossae
  1. Anterior Cranial Fossa: Formed by the frontal bone and part of the ethmoid bone. It supports the frontal lobes of the brain.

  2. Middle Cranial Fossa: Formed by the sphenoid and temporal bones, houses the temporal lobes and the pituitary gland (within the sella turcica).

  3. Posterior Cranial Fossa: Formed by the occipital and temporal bones; contains the cerebellum, pons, and medulla oblongata.

Key Bones and Their Features
  • Frontal Bone: Comprises three parts: squamous, nasal, and orbital. The squamous part forms the forehead and includes structures like the supraorbital notch for blood vessels and nerves.

  • Occipital Bone: Has four parts and features like the foramen magnum for the brainstem and vertebral arteries.

  • Sphenoid Bone: A bat-shaped bone that contributes to the skull base and has the sella turcica for the pituitary gland.

Mandible and Temporomandibular Joint (TMJ)

The mandible is the only movable bone in the skull and is articulated with the skull via the TMJ, which functions as a modified hinge joint. It allows for various movements such as elevation, depression, protrusion, retrusion, and lateral movements, facilitated by the muscles of mastication (masseter, temporalis, lateral and medial pterygoids).

Nasal Cavity and Paranasal Sinuses

The nasal cavity, formed from several bones (including the maxilla and ethmoid), is divided into sections by conchae, which increase surface area for warming and olfaction. Paranasal sinuses (frontal, maxillary, ethmoidal, and sphenoidal) extend from the nasal cavity and contribute to the resonance of the voice as well as the humidification and warming of inspired air.

Cranial Nerves

The skull houses and protects the cranial nerves:

  1. CNI - Olfactory nerve (smell)

  2. CNII - Optic nerve (vision)

  3. CNIII - Oculomotor (eye movement)

  4. CNIV - Trochlear (eye movement)

  5. CNV - Trigeminal (sensory and motor functions of face)

  6. CNVI - Abducens (eye movement)

  7. CNVII - Facial (muscles of facial expression)

  8. CNVIII - Vestibulocochlear (hearing and balance)

  9. CNIX - Glossopharyngeal (taste and gag reflex)

  10. CNX - Vagus (autonomic functions)

  11. CNXI - Accessory (shoulder movement)

  12. CNXII - Hypoglossal (tongue movement)

These nerves exit the skull through various foramina and canals, each serving specific areas of the face and neck.

Clinical Significance

Understanding the anatomical features of the skull is essential for diagnosing and treating various conditions, as well as for conducting surgical procedures in craniofacial surgery and neurosurgery. Pathological conditions, such as skull fractures or traumatic brain injuries, require a thorough understanding of skull anatomy for appropriate management.
