Principal Display Panel

1. Product Identity

   • Name of the product

   • Statements of identity

   • Net contents

   • Warning: "The safety of this product has not been determined" (if applicable)

2. Safety Testing

   • Determine adequacy of toxicological and other safety testing (ref: 21 CFR 740.10)

Information Panel

1. Manufacturer Information

   • Name and address of the firm manufacturing or introducing the product

2. Ingredient List

   • Required on outer container if intended for consumer sale

3. Warnings and Directions

   • Warning statements as per 21 CFR 740.11, 740.12, and 740.17

   • Additional necessary warnings for health hazards

   • Safe use directions

   • For hair dyes: caution statement per Sec. 601(a) of the Act, including directions for preliminary patch testing (only for coal-tar hair dyes exempted from adulteration provisions)
