Cavities and Planes: Key Concepts for Anatomy & Physiology
Overview: the body is organized into cavities, planes, membranes, and regional divisions that help locate organs and describe relationships.
Two major body cavity groups (major divisions):
- Dorsal cavity: contains the brain and spinal cord. This includes the cranial cavity and the spinal cavity. Anatomical terms often pair with the back surface.
- Ventral cavity: in front of the body; subdivides into the thoracic cavity and the abdominopelvic (abdominal + pelvic) cavity. The diaphragm forms the boundary between the thoracic and abdominal/pelvic regions.
- Quick relation: dorsal vs ventral = back vs front. The dorsal cavity is back-ward; the ventral cavity is toward the front.
- There are two main ventral/dorsal groupings that work together to house all internal organs.
Thoracic cavity and its subdivisions:
- The thoracic cavity is the upper ventral cavity region that houses:
- Pleural cavities surrounding the lungs.
- Pericardial cavity surrounding the heart.
- Mediastinum: the central area between the lungs containing the heart and other structures.
- The diaphragm is the bowl-shaped muscle that forms the boundary between the thoracic cavity and the abdominal/pelvic cavity.
Abdominopelvic (abdominal + pelvic) cavity:
- Encloses most of the digestive organs and other organs; separated from the thorax by the diaphragm.
- Organs within include the stomach, pancreas, liver, gallbladder, small intestine, large intestine, kidneys, adrenal glands, uterus (in females), bladder, and more.
- The peritoneum surrounds many abdominal organs; kidneys are retroperitoneal (behind the peritoneum).
Nine abdominal regions (vs. quadrants) for localization:
- The central vertical line runs through the umbilicus (navel) as a reference point.
- Regions around the umbilical region include:
- Epigastric region (above the umbilicus).
- Umbilical region (around the belly button).
- Hypogastric (pubic) region (below the umbilicus).
- Lateral regions include:
- Right hypochondriac, Left hypochondriac (by the rib cage).
- Right lumbar, Left lumbar.
- Right inguinal (iliac), Left inguinal.
- Regional names and prefixes:
- epi- means above or on the surface; epigastric is above the stomach region.
- hypo- means below; hypogastric is below the umbilical region and near the pubic area.
- hypochondriac refers to below the cartilage of the ribs.
- All together: there are regions in this scheme used to localize organs more precisely than the four-quadrant approach.
Planes and body sections (orienting by anatomical position):
- Sagittal cuts: cut along the length of the body; vertical plane.
- Midsagittal (median) cut: exactly down the middle.
- Parasagittal cut: off-center, lateral to the midline.
- Transverse (horizontal) cuts: cut perpendicular to the long axis; divides superior from inferior parts.
- Frontal (coronal) cuts: cut along a plane that divides anterior (front) from posterior (back);
- Coronal sections are also called frontal sections.
- The anatomical position is always the frame of reference for describing these cuts.
Dorsal cavity and ventral cavity details: what’s inside and how they relate
- Dorsal cavity contents:
- Cranial cavity: contains the brain.
- Spinal cavity: contains the spinal cord.
- Ventral cavity contents (overview):
- Thoracic cavity: includes pleural cavities (lungs), pericardial cavity (heart), and mediastinum.
- Abdominopelvic cavity: contains the abdominal organs (stomach, intestines, liver, pancreas, gallbladder, etc.) and pelvic organs (bladder, reproductive organs, distal portions of the large intestine).
- Key boundary: the diaphragm separates the thoracic cavity from the abdominal/pelvic cavity.
Membranes and serous layers: visceral vs parietal
- Serous membranes line cavities and cover organs, creating a potential space (serous cavity) between layers.
- Parietal layer: lines the outer wall of a cavity (surface away from the organ).
- Visceral layer: directly covers the organ.
- Examples:
- Pleura: visceral pleura covers the lungs; parietal pleura lines the thoracic cavity.
- Pericardium: visceral pericardium covers the heart; parietal pericardium lines the pericardial cavity.
- Peritoneum: visceral peritoneum covers abdominal organs; parietal peritoneum lines the abdominal cavity.
- Important note: the pleura is the exception that does not carry the “peri-” prefix in its common name, but pericardium and peritoneum do include “peri-” (around).
- Space between parietal and visceral layers forms the cavity that holds the lubricating fluid.
Organs and relationships in the thorax and abdomen
- Trachea vs esophagus:
- Trachea (air passage) lies anterior to the esophagus.
- Esophagus runs posterior to the trachea as it descends toward the stomach.
- Mediastinum houses the heart and other structures; the heart sits centrally between the lungs.
- Lungs are enclosed by pleural cavities; heart is enclosed by the pericardium.
Abdominal organs and supporting structures (location and names)
- Stomach: gastric region; often paired with the epigastric region.
- Pancreas: often described as a “chicken finger” shape in many models.
- Liver: a large organ in the upper right quadrant; bile is produced here.
- Gallbladder: a green sac that stores bile produced by the liver; bile aids digestion of fats; gallbladder stores and concentrates bile; bile release can be reduced if gallbladder is removed.
- Small intestine: compact within the abdominal cavity; numerous loops; site of most nutrient absorption.
- Large intestine: surrounds the small intestine; absorbs water and forms feces; includes regions such as the cecum, colon, and rectum (not all named in the lecture, but the relationship is described as surrounding the small intestine).
- Appendix: small projection at the beginning of the large intestine; frequently removed for appendicitis; its immune function is debated.
- Kidneys: retroperitoneal (behind the peritoneum) and located posterior to the intestines; adrenal glands sit atop the kidneys.
- Adrenal glands: small glands atop the kidneys; secrete hormones involved in stress response and other physiological processes.
- Ureters: tubes transporting urine from the kidneys to the bladder.
- Bladder: stores urine; located in the pelvic region.
- Peritoneum: membrane that lines the abdominal cavity and covers many abdominal organs.
Retroperitoneal structures and terminology
- Retroperitoneal means behind the peritoneum (e.g., kidneys).
- This term helps distinguish organs that lie behind the peritoneal lining from those that are intraperitoneal (within the peritoneal cavity and covered by peritoneum).
Brain stem and cavity placement (Q&A example)
- Question: Is the brain stem part of the cranial cavity or the spinal cavity?
- Answer: Primarily cranial cavity; the brain stem is located within the cranial cavity, though it forms a transitional region near the boundary with the spinal cord.
Practical notes for students
- Always break down medical terms into prefixes and suffixes to understand meaning (epi-, hypo-, peri-, etc.).
- Use the anatomical position as your reference frame when analyzing cuts and sectional views.
