Mental Models and Situational Awareness for Emergency Responders
Foundations of Mental Models
- Definition of Mental Models: Mental models serve as the human mind's point of reference regarding current occurrences and future projections. At an emergency scene, every responder develops these models based on various factors.
- Factors Influencing Mental Models:
- Collective Training and Experience: The sum of a responder's professional background.
- Knowledge of the Emergency Type: Specific information regarding the nature of the crisis, including details about buildings or structures involved.
- Dispatch Information: Data provided by communications centers prior to arrival.
- Size-up Information: Observations made by the responder upon reaching the scene.
- Goal of Mental Models: Combined, these factors help a responder understand "what is happening now" and, crucially, predict "what is going to happen next."
Mental Models and Situational Awareness (SA)
- Rooting in SA: Mental models are fundamentally rooted in situational awareness. The transcript references three levels of situational awareness:
- Level 1 SA: Capturing the clues and cues occurring in the current environment.
- Level 2 SA: Processing those specific clues and cues to derive meaning.
- Level 3 SA: Making accurate predictions of future events.
- Recognition-Primed Decision Making (RPDM):
- Mental models are a critical component of RPDM.
- When the brain identifies a "pattern match" based on past experiences, it can trigger an intuition-based response.
- Intuition and Tacit Knowledge: Intuition is often described as a "gut feeling" that the responder may not be able to explain logically, as it originates from tacit (subconscious) knowledge.
The Limitations and Flaws of Intuition
- Lack of Flawlessness: Intuition derived from tacit knowledge is not perfect. Two major risks exist:
- The brain may fail to make a pattern match, resulting in no intuitive guidance.
- The intuition or gut feeling provided by the brain may be factually wrong.
- Synthesized Pattern Matching: A responder does not need to have experienced an exact scenario to benefit from a pattern match. The brain can take "bits and pieces" of various different past experiences (stored as patterns) and assemble them like a "proverbial mental puzzle" to form a new match.
- Neuroscience Perspective: Intuition is sometimes referred to as "unconscious decision making."
- The Problem of Incomplete Inputs: Because the brain forms matches based on processed clues and cues, missing information is equivalent to missing pieces of a puzzle. If the brain assigns the wrong meaning to a cue or processes a cue incorrectly, it becomes confused.
Physiological and Cognitive Blind Spots
- The Visual Blind Spot: Physiologically, there is a small area in the back of the eye lacking photon (light) receptor nerve cells. This creates a literal hole in vision.
- Brain Hallucination: The brain fills in this visual hole based on memory of past experiences and calculations of current surroundings. Essentially, the brain "imagines" what should be there.
- Real-world Implication: In driving, a car may be in a visual blind spot. The brain might fill that spot with an image of an open roadway or empty intersection, leading to accidents where the driver claims, "I didn't even see the other car."
- Cognitive Filling: Just as it does with vision, when faced with incomplete situational data, the brain fills in facts based on what it "thinks" the facts should be. This leads to intuition based on incomplete or "hallucinated" inputs.
The Hallucination Test Procedure
- Objective: To demonstrate the brain's ability to fill in missing information.
- Materials: A blank index card.
- Setup:
- On the left side, draw a dot the size of a pea.
- On the right side, draw an "X" of the same size.
- Execution Steps:
- Hold the card in the outstretched left hand with the "X" on the right side.
- Keep both eyes open, but use the right hand to completely cover the right eye.
- Look at the "X" using only the left eye.
- Note that the dot is still visible in the peripheral vision.
- Slowly draw the card toward the face while maintaining focus on the "X."
- Result: At a specific point, the dot will disappear. This indicates the blind spot. If it does not happen, the transcript jokingly suggests the person might be an "anatomical freak of nature."
Strategies to Improve Situational Awareness
- Three Requirements for Decision Makers:
- Be Conscious: Being awake and alert.
- Be Present: This can mean physical presence at the scene or being mentally present via radio traffic to capture clues and cues.
- Scan the Environment: This must be done both visually and audibly.
- Purpose of Environmental Scanning:
- Avoiding fixation on a single task or geographical area.
- Filling in blind spots with accurate, real-time information rather than imagined data.
Trust, But Verify
- Validation of Intuition: Responders should search for facts that validate the accuracy of their intuition. Facts are the evidence the rational brain uses to confirm the veracity of a gut feeling.
- The Reagan Rule: Borrowing from President Ronald Reagan regarding nuclear arms reduction: "Trust, but verify." This is critical for managing intuitive feelings under stress.
- The "Stubborn Brain": Once the brain locks onto a belief, it is difficult to change. The brain may dismiss contrary facts because they do not fit the existing understanding of the situation. It may even invent excuses for why new, accurate information is wrong.
- Confirmation Bias: The brain can unknowingly deploy a vetting process that seeks out only those facts that support existing beliefs. This is summarized as: "We see what we want to see."
Challenge and Verification Processes
- The Devil's Advocate: Responders should actively ask how odd clues or cues might mean something different than their current theory.
- The Command Aide/Advisor: Ideally, a separate person should challenge and verify the mental model of the Incident Commander. This role provides a second set of eyes and ears. Discrepancies between the aide and the commander warrant further evaluation.
- Moderation in Challenge: This process must be done in moderation. Excessive "badgering" or running long lists of "what-if" scenarios can choke a decision maker's capacity to implement a timely action plan.
Pre-arrival Mental Models
- Formation: Models are often formed before arrival by combining current call information with memories of similar past calls or previous history at that specific address.
- Preparation Strategy: A common recommendation for young responders is to mentally prepare for the worst possible scenario. This ensures preparation for extreme challenges while providing a sense of calm if the scene is less severe than imagined.
- Potential Danger: Forming a mental model too early can lead to making decisions or giving orders before the real incident is actually observed.
- Fantasized Reality: The brain has difficulty distinguishing vivid imagination from facts. If a responder arrives and the real scene differs from their pre-formed "fantasy," they may struggle to change their mind and remain committed to the flawed, imagined model.
