Psychiatry: 1. Intro and Mental State examination

INTRODUCTION TO PSYCHIATRY

  • Focus on understanding, assessment, diagnosis, and treatment of mental disorders.

LEARNING OUTCOMES

  • Describe common psychiatric disorders that can present as physical illnesses with reference to conditions taught in FFP1

  • Develop an appreciation of where psychiatry interfaces with conditions taught in FFP1

  • Outline the impact of the individual's environment or system on their physical and mental health using the biopsychosocial approach

  • Outline the components of a mental state examination

  • Describe how engaging effectively with patients, colleagues and communities requires managing the self and self-care, using a biopsychosocial approach with specific reference to stress

  • Describe combined disease burden and outcomes in comorbid physical and mental illness

COMMON MENTAL DISORDERS

  • Common disorders include depression and anxiety, manageable in primary care.

  • Symptoms of depression: persistent low mood, biological changes (sleep, energy), and cognitive alterations (hopelessness).

  • Anxiety symptoms include persistent worry, biological symptoms (fatigue, sleep disruption), and cognitive issues (impaired attention).

INTERFACE WITH PHYSICAL ILLNESS

  • Depression and physical conditions (e.g., anemia, inflammation) can mimic each other.

  • Sepsis can influence mental state through inflammation and hypoxia.

  • Recognize bidirectional effects of mental and physical health.

MENTAL STATE EXAMINATION (MSE)

  • Assesses thoughts, emotions, and behaviors; identifies mental health conditions.

  • Key components: appearance, behavior, speech, mood, thought form/content, perception, cognition, insight & judgment.

BIOPSYCHOSOCIAL APPROACH

  • Health influenced by physical environment, social context, and psychological factors.

BURDEN OF DISEASE

  • 1 in 8 people globally have a mental disorder.

  • Significant increase in anxiety and depression disorders post-pandemic.

  • Higher rates of poor mental health in chronic physical conditions.

  • Comorbidity leads to increased morbidities and costs, often underrecognized.
