Overview and Structure of the Diencephalon

  • Definition and Location:

    • The diencephalon is the central core of the interbrain.

  • Five Major Parts of the Diencephalon:

    • Epithalamus: Positioned dorsomedial to the thalamus.
      • Components: Habenular nucleus, Habenular commissure, Pineal gland, and Posterior commissure.
    • Subthalamus: Located below the posterior parts of the thalamus.
      • Components: Subthalamic nuclei, rostral extension of the red nucleus, and substantia nigra (which extends from the midbrain).
    • Metathalamus: Situated beneath the caudal end of the thalamus.

Gross Features and Relations of the Thalamus

  • General Characteristics:

    • A large mass of grey matter situated in the lateral wall of the third ventricle.
    • Functionally acts as the primary relay station for sensory impulses.
    • Transfers impulses to specific cortex areas for localization and interpretation.
  • Ends and Surfaces:

    • Anterior End: Forms the posterior boundary of the interventricular foramen.
    • Posterior End (Pulvinar): Overhangs the lateral and medial geniculate bodies.
    • Superior Surface: Related to the floor of the lateral ventricle and the tela choroidea of the third ventricle.
    • Inferior Surface: Rests upon the subthalamus and hypothalamus.
    • Medial Surface: Forms the lateral wall of the third ventricle.
    • Lateral Surface: Forms the medial boundary of the posterior limb of the internal capsule.
  • Other Related Anatomical Landmarks:

    • Interthalamic adhesion.

Functional Organization and Nuclei of the Thalamus

  • Core Thalamic Functions:

    • Main relay center between the cerebral cortex and lower structures.
    • Relays information from: basal ganglia, brainstem, spinal cord, and cerebellum.
    • Relay for all sensory systems except the olfactory system.
  • Three Functional Classes of Nuclei:

    1. Specific Sensory Relay Nuclei: Process specific sensory modalities.
    2. Motor Nuclei: Interconnect frontal cortices with the cerebellum and basal ganglia.
    3. Nonspecific Association Nuclei: Interconnect cortical association regions with the limbic system.

    • Connections: Receives inputs from the Mamillary body; projects to the Cingulate gyrus.

  • Medial Nuclear Group (Mediodorsal/Dorsomedial Nucleus):

Clinical Syndromes of the Thalamus

  • Thalamic Syndrome (Dejerine-Roussy Syndrome):

    • Cause: Hemorrhage or infarction, usually involving VPLVPL and VPMVPM nuclei.
    • Symptoms:
      • Sensory disturbance: Diminished somatic sensation on the contralateral side; threshold for touch, pain, and temperature is raised.
      • Motor deficits: Delayed rigidity and choreodystonic involuntary movements.
      • Psychological problems.
  • Thalamic Hand:

    • Physical orientation: Wrist is pronated and flexed; metacarpophalangeal joints are flexed while proximal and distal interphalangeal joints are extended.
  • Thalamic Phantom Limb:

    • A condition where the patient, with eyes closed, is unable to locate the position of their limb.
  • Specific Nuclear Lesions:

    • Anterior Nuclei: Loss of recent memory (Korsakoff’s syndrome / Amnesia).
    • Centromedian Nucleus: Causes Thalamic pain.
    • Ventro-anterior Nucleus: Associated with Parkinsonism.
  • Regional Syndromes:

    • Medial: Disorders of consciousness (thalamic amnesia, akinetic mutism).

    • Habenular Nuclei/Trigonum: Part of the limbic system. Receives olfactory, lateral hypothalamus, and amygdaloid complex afferents. Regulates food and water intake.
    • Stria Medullaris thalami.
  • Subthalamus Details:

    • Location: Between the midbrain and thalamus; medial to the internal capsule and globus pallidus.
    • Grey Matter:
      • Subthalamic nucleus (important for motor integration/movements).
      • Zona incerta (thin lamina between thalamus and subthalamus, continuous with reticular nucleus; function unknown).
      • Cranial ends of the red nucleus and substantia nigra.
    • White Matter / Fields of Forel:
      • H2H2 FF: Globus to thalamus.
      • H1H1 FF: Ansa lenticularis to form the thalamic fasciculus.
      • Contains cranial ends of lemnisci, dentato-thalamic, and rubrothalamic tracts.

The Hypothalamus: Structure and Zonal Anatomy

  • Location: Under the thalamus, forming the floor and lateral wall of the third ventricle.

  • Anatomical Borders/Subdivisions:

    1. Preoptic Region: Derived from the telencephalon impar; located along the lamina terminalis.
    2. Supraoptic Region: Above the optic chiasma.
    3. Tuberal Region: Infundibulotuberal area.
    4. Mamillary Region: Includes the mamillary bodies.
  • Medial Zone Nuclei:

    • Preoptic, Paraventricular, Dorsomedial, Ventromedial, Infundibular (Arcuate), and Posterior nuclei.
  • Lateral Zone Nuclei:

    • Supraoptic, Lateral tuberal, Tuberomamillary, and Lateral nuclei.

Hypothalamic Nuclei: Specific Features

  • Supraoptic Nucleus (SONSON): Large cells at the junction of the optic chiasma and tract.
  • Paraventricular Nucleus (PVNPVN): Forms a vertical plate of cells; axons form the hypothalamo-hypophysial tract. Contains corticotropin-releasing hormones.
  • Suprachiasmatic Nucleus (SCNSCN): Located above the optic chiasma. Operates as a luminance detector (day/night sensor) for circadian rhythms.
  • Ventromedial Nucleus: Oval-shaped.
  • Arcuate (Infundibular) Nucleus: Located in the floor of the third ventricle; contains GHRHGHRH (Growth Hormone Releasing Hormone).
  • Lateral Hypothalamic Nucleus: Regulates sympathetic activity.
  • Mamillary Bodies: Receive fornix terminations; originate the mamillothalamic and mamillotegmental tracts.

Physiological Functions of the Hypothalamus

  • Autonomic Center: Regulates sympathetic and parasympathetic activities.
  • Endocrine Control: Secretes hormones influencing the Pituitary gland, which in turn controls the Thyroid, Adrenal glands, and Gonads.
  • Temperature Regulation:
    • Anterior Part: Cooling/heat dissipation.
    • Posterior Part: Heat conservation/arousal center.
  • Food and Water Intake:
    • Feeding Center: Lateral eminence.
    • Satiety Center: Medial eminence.
  • Circadian Rhythm: Managed by the SCNSCN; governs the sleep-wake cycle and paradoxical sleep.
  • Limbic Role: Influences emotions and behavioral status.

Clinical Disorders of the Hypothalamus

  • General Causes: Inflammation, neoplasm, vascular disorders, or surgical damage.

  • Obesity and Wasting: Result from lesions in satiety or feeding centers.

  • Temperature Disturbances:

    • Hyperthermia: Lesion of the anterior hypothalamus.
    • Hypothermia: Lesion of the posterior hypothalamus.
  • Diabetes Insipidus:

    • Cause: Destruction of the Supraoptic nucleus or interruption of the hypothalamo-hypophysial tract.
    • Symptoms: Excessive thirst, increased water intake (polydipsia), and polyuria.
  • Sleep Disorders:

    • Narcolepsy: Abnormal sleep patterns/short periods of sleep during waking hours.
    • Cataplexy: Sudden emotional outbursts (anger, fear, excitement) associated with narcolepsy.
  • Other: Sexual disorders (retardation or precocity in children) and emotional disorders.
