Key Characteristics of Trematodes

  • Non-segmented bodies, unlike cestodes which have segmented bodies.
  • Growth can result in sizes measurable in meters.
  • Morphology includes:
    • Flat bodies with bilateral symmetry.
    • Presence of suckers: oral suckers (for nourishment) and ventral suckers (for attachment).
  • Require two or more hosts for their life cycles, with snails commonly serving as the first intermediate host.
  • Humans act as accidental definitive hosts, not intermediate hosts.

Classification of Flukes

  • Flukes can be classified based on:
    • Reproduction:
    • Most are hermaphroditic (monoecious), meaning they have both male and female reproductive organs.
    • Schistosoma is an example of a dioecious group (separate sexes).

    • Various species inhabit lungs, liver, or intestines.
    • Schistosoma generally affects blood.

Life Cycle of Trematodes

  1. Fertilized Egg: Must contact water to embryonate.
  2. Embryonated Egg: Develops in water, leading to the first larval stage.
  3. Larval Stages:
    • Miracidium: Ciliated and elongated.
    • Develops into Sporocyst: a sac-like structure.
    • Special larval forms include Cercaria: capable of skin penetration, moving to the second intermediate host.
    • Final larval form Metacercaria: infective stage for definitive host (usually a human).

Specific Types of Flukes

1. Paragonimus westermani (Oriental Fluke)
  • Causative agent of paragonimiasis (pulmonary disease).
  • Laboratory diagnosis via sputum examination, rarely feces.
  • Intermediate hosts: freshwater crabs or crayfish.
  • Morphological features:
    • Broadly oval shape with opercular features.
    • Zigzag intestinal ceca; adults resemble coffee beans.
  • Clinical manifestations can mimic tuberculosis, leading to hemoptysis and potential brain migration.
2. Fasciola hepatica (Sheep Liver Fluke)
  • Causative agent of fascioliasis (sheep liver rot).
  • Laboratory diagnosis includes stool examination and serological tests.
  • Intermediate host: water plants like watercress.
  • Morphological details:
    • Thin shell with small operculum and irregular ends.
