Blood Glucose Testing Procedure
Initial Assessment and Preparation
- Inspect the patient's fingers and forearms for edema, inflammation, cuts, and sores.
- Avoid areas with bruises or open lesions.
- Position the patient comfortably in a chair or in a semi-Fowler's position in bed.
- Wash hands before proceeding with the procedure.
Blood Glucose Testing Equipment
- Acknowledge the variety of blood glucose meters available.
- Emphasize the importance of following the manufacturer's specific instructions for the particular model being used.
Procedure
- Remove the reagent strip from the vial and tightly reseal the cap to protect the remaining strips from humidity and environmental factors.
- Insert the test strip into the meter, taking care not to bend it, as this could damage the strip or the meter.
- The meter should turn on automatically. If it doesn't, consult the meter's manual for instructions on how to power it on.
- Check the code on the vial to ensure it matches the code on the meter (if required by the meter type). This ensures accurate readings.
- Use only the test strips recommended for the specific glucose meter you are using to avoid inaccurate results.
- Note that some newer meters do not require coding, while others use a disc or drum containing multiple strips for convenience.
Lancet Device Preparation
- Prepare a single-use or multiple-use lancet device. Ensure the device is clean and ready for use.
- Remove and replace the cap on the device to prepare it for puncturing the skin.
- Cock the lancet device to activate it. This mechanism readies the needle for a quick and controlled puncture.
- Adjust the puncture depth on the lancet device according to the patient's skin thickness and sensitivity.
Selecting and Preparing the Puncture Site
- Apply clean gloves to maintain hygiene and prevent infection.
- Select a puncture site in a vascular area to ensure an adequate blood sample. The lateral side of the finger is usually a good choice for stable adults.
- Avoid the central tip of the finger due to its denser nerve supply, which can make the procedure more painful.
- Clean the selected site on the patient's finger with an antiseptic swab to prevent infection.
- Wipe the puncture site lightly to avoid causing irritation or discomfort.
Puncture and Blood Sample Collection
- Hold the area of the puncture site in a dependent position to encourage blood flow to the fingertip.
- Avoid milking or massaging the finger excessively, as this can alter the composition of the blood sample and affect the accuracy of the glucose reading.
- Hold the tip of the lancet against the skin at the intended puncture site and press the release button to create the puncture.
- Some lancet devices allow you to see the blood sample forming. This helps ensure that you obtain an adequate sample.
- Remove the device after the puncture.
- If you do not see a drop of blood, gently squeeze or massage the fingertip until a blood drop forms. Avoid excessive squeezing.
- Ensure the meter is still on and bring the meter with the test strip already in place to the finger.
- Wipe the blood onto the test strip, following the specific instructions for the meter being used.
- Ensure an adequate blood sample is obtained, as insufficient blood can lead to inaccurate readings.
- The test result will appear on the screen of the meter.
- Some devices emit a beep when the blood glucose measurement has been made, indicating that the reading is complete.
Completion and Follow-Up
- Turn off the meter if your model doesn't turn off automatically to conserve battery power.
- Dispose of the test strip and lancet in the proper receptacles (sharps container) to prevent accidental needlesticks and ensure safe disposal.
- Remove and dispose of your gloves, following infection control protocols.
- Perform hand hygiene to further prevent the spread of infection.
- Assess the puncture site for any bleeding and apply a bandage if necessary.
- Discuss the test results with the patient and give her a chance to ask questions and express concerns.
- Encourage the patient to become an active participant in her own care, especially if newly diagnosed with diabetes mellitus. This can improve their understanding and adherence to treatment plans.
- Explain sliding scales for insulin administration based on blood sugar levels; for example, if blood sugar is 200, administer a certain amount of insulin as per the prescribed sliding scale.
