GP1-Risk and Uncertainty

Risk likelihood of harm occuring

Uncertain is not able to be relied on; not known or definite

Uncertainty is state of being not completely sure or confident of something

Roger neighbour’s Inner Consultation Model

1-connecting with your patient

2-summarising for you and patient, patient understands you were listening, patient has opportunity to correct anything you misunderstood or to add something

3-handover, making sure patient is happy and agrees with management plan like gift wrapping and giving back to patient

4-safety netting, communicates to patient

5-housekeeping, to keep yourself in a good frame of mind to deal with next patient, documentation and records, if you’re gonna refer patient or not

Risk is an individual experience and perception of risk varies from person to person

Individualising care is very important

Problem Solving

Diagnosis requires -systematic enquiry -full examination

Hypothetico-deductive reasoning (HDR) is used

HDR in primary care

-make a few initial diagnostic hypotheses

-narrow down possible diagnoses through focused history and examination

-review diagnosis in light of new information eg progression of symptom or test results
