The Nursing Process

Steps of the Nursing Process for Registered Nurses (RNs)

  1. Assessment

    • Collect both objective and subjective data pertaining to the client.

  2. Analysis

    • Identify client problems based on collected data.

  3. Planning

    • Formulate a care plan to address identified problems.

  4. Implementation

    • Execute the care plan by providing necessary interventions.

  5. Evaluation

    • Assess the effectiveness of the interventions and document the client’s responses.

Steps of the Nursing Process for Licensed Practical Nurses (LPNs)

  1. Data Collection

    • Gather subjective and objective data and communicate changes to the RN.

  2. Planning

    • Develop a care plan under the RN's supervision.

  3. Implementation

    • Collaborate with the RN to carry out the plan.

  4. Evaluation

    • Review the interventions' effectiveness, within the LPN's scope and under RN supervision.

Nursing Process Dynamics

  • The steps of the nursing process typically proceed in a linear fashion.

  • However, it is common to revisit steps (particularly planning, implementation, and evaluation) to optimize client outcomes.

Critical Thinking in Nursing

  • Definition: The ability to analyze and interpret data to address problems and achieve outcomes.

Clinical Judgment

  • Definition: An ongoing process of clinical reasoning that evolves with experience.

  • It is the outcome of critical thinking and encompasses:

    • Nursing knowledge

    • Recognition of client situations

    • Prioritization of issues

    • Evidence-based practice utilization.

Important Note for Nurses

  • Clinical reasoning and judgment are tasks that cannot be delegated to others.
