The Patient

  • Sedation Needs:

    • Children often require sedation or anesthesia for various procedures.

  • Considerations for Comorbidities:

    • Patients with significant comorbidities may not tolerate major surgical procedures; NORA can be a viable option.

  • Pre-Anesthesia Assessment:

    • All patients require a thorough assessment and a tailored anesthetic plan, alongside appropriate monitoring.

The Procedure

  • Anesthesiologist's Understanding:

    • Critical to grasp details such as patient positioning, pain levels, duration of the procedure, and any special requirements (e.g. contrast media).

Anesthesia Reversal

  • Definition:

    • The process of counteracting anesthetic drug effects to restore consciousness and normal physiology.

  • Common Antagonists:

    • Naloxone (for opioids)

    • Flumazenil (for benzodiazepines)

  • Stimulants Used:

    • Neostigmine (for neuromuscular blockers)

The Environment

  • Challenges in NORA Settings:

    1. Limited Access:

    • Access may be restricted by diagnostic equipment (e.g., CT, MRI).

    1. Unique Hazards:

    • Hazards like radiation (CT, fluoroscopy) and magnetic fields (MRI) can be present.

    1. Staff Familiarity:

    • Medical staff at these locations may lack familiarity with anesthesia requirements.

    1. Emergency Assistance:

    • Quick access to anesthesia colleagues in emergencies may be limited.

Standards of Care

  • Development by ASA:

    • The American Society of Anesthesiologists (ASA) has set standards for NORA.

    • Many NORA procedures involve sedation or monitored anesthesia care.

  • Risks of Deep Sedation:

    • Deepening sedation can lead to loss of airway reflexes, potential airway obstruction, and respiratory depression.

  • Monitoring and Recovery:

    • Patients drifting deeper than intended require transfer to a recovery area with equivalent standards to postoperative care.

Adverse Events

  • Rarity of Significant Events:

    • Serious adverse events in NORA are infrequent; however, mortality rates are higher than in operating room anesthesia.

  • Common Complications:

    • Common issues include airway obstruction and respiratory depression due to oversedation, notably in patients with obstructive sleep apnea.
