Gastroesophageal Reflux Disease (GERD)
Overview
GERD is the most common benign condition affecting the stomach and esophagus. It occurs due to the failure of antireflux mechanisms, primarily involving the lower esophageal sphincter (LES).
Surgical intervention, particularly laparoscopic anti-reflux surgery (LARS), is recommended for patients with persistent symptoms despite maximal medical therapy.
Laparoscopy has been shown to reduce morbidity, hospital stays, and costs compared to open surgery.
Pathophysiology
The LES prevents gastric contents from refluxing into the esophagus. Its function relies on several structures:
Intrinsic Musculature: Maintains tonic contraction and relaxes briefly to allow swallowing.
Sling Fibers of the Gastric Cardia: Assist in maintaining the high-pressure zone of the LES.
Crura of the Diaphragm: Contributes to LES pressure via changes in thoracic and abdominal pressure during breathing.
Gastroesophageal Junction (GEJ): Proper anchoring in the abdominal cavity is crucial for avoiding reflux.
GER manifests when intragastric pressure exceeds LES pressure, leading to symptomatic reflux.
Diagnosis and Evaluation
Diagnosis typically involves assessing typical symptoms and may also include:
Ambulatory pH Monitoring: The gold standard for diagnosing GERD by quantifying acid exposure.
Esophageal Manometry: Evaluates the functional status of the esophagus and measures LES pressure.
Esophagogastroduodenoscopy (EGD): Identifies mucosal injury due to GERD and allows for direct imaging.
Treatment
Medical Management: Proton Pump Inhibitors (PPIs) are the first line of treatment. Patients should undergo an 8-week trial of PPIs.
Surgical Treatment: For those unresponsive to PPIs, LARS is an effective option, providing durable relief and is cost-effective compared to prolonged medical therapy.
Hiatal Hernia
Types of Hiatal Hernias
Sliding Hiatal Hernia (Type I): The most common type; occurs when the GEJ migrates into the thorax.
Paraesophageal Hernia (Type II-IV): Involves the stomach sliding through the diaphragm but retains GEJ position, often leading to more severe symptoms.
Type II: GEJ remains anchored, with the fundus herniating.
Type III: Both GEJ and fundus are in the thorax.
Type IV: Other visceral organs may also herniate into the mediastinum.
Clinical Presentation
Symptoms can range from mild GERD to severe obstructive symptoms, including dysphagia and acute complications such as gastric volvulus.
Preoperative Evaluation
Similar diagnostic tests are utilized for PEH. Barium studies help visualize the anatomy, while endoscopy assesses mucosal injuries.
Surgery Performance
Repair can be performed via laparoscopy and involves:
Hernia content reduction and sac excision.
Mobilization of the esophagus to obtain adequate intra-abdominal length.
Closure of the hiatus, often with reinforcement using biologic mesh.
Complications and Management
Postoperative complications may include dysphagia, bloating, and potential herniation. Close monitoring and early intervention for complications are crucial.
Summary of Treatment Approaches
Comprehensive approaches are necessary to evaluate and treat patients with GERD and hiatal hernia effectively. The surgical management offers significant benefits, particularly in symptomatic relief and quality of life improvements.
Ongoing research is required to optimize techniques and explore alternative therapies like magnetic sphincter augmentation (LINX) for those not suitable for traditional surgery.
