Fundamentals of the Integumentary System

The skin is defined as the largest organ of the human body. It is structurally organized into 22 primary layers. The superficial layer is known as the Epidermis, which is composed of stratified squamous epithelial tissue. Beneath this lies the deep layer, known as the Dermis, which consists of loose or areolar connective tissue. While not considered a formal component of the skin itself, the hypodermis is an underlying layer of adipose connective tissue that serves the critical function of attaching the skin to the rest of the body.

Anatomy and Stratification of the Epidermis

The epidermis is composed of either 44 or 55 distinct layers, also referred to as strata. The deepest of these layers is the stratum basale, which consists of simple cubodial epithelial tissue. This layer is characterized by active cellular division, which is essential for the production of new epidermis. Cells within the epidermis contain internal structures such as the nucleus and the golgi apparatus. Specialized cells known as melanocytes are present, which contain melanosomes. External to the stratum basale are the stratum spinosum and the stratum granulosum.

In specific regions of the body characterized as thick skin, an additional layer called the stratum lucidum is found. The most superficial layer is the stratum corneum, which is made up of dead cells filled with keratin. These cells eventually exfoliate from the surface of the body.

Characteristics of the Dermis and Cutaneous Glands

The dermis is frequently referred to as the true skin. Within the dermis are two primary types of cutaneous glands that facilitate different physiological processes. Sebaceous Glands are responsible for the production of sebum, an oily substance. Sweat Glands are further categorized into two types: Apocrine and Metocrine. The dermis also contains the nerve endings necessary for sensation, a function shared with the stratum basale.

Structural Components of Hair and Follicles

Hair is classified into 33 specific types: lanugo, vellus, and terminal. The anatomical structure of a hair consists of an inner medulla, a cortex, and a cuticle. Hair follicles house the hair root and are formed by the stratum basale. At the base of the follicle is the dermal papilla, which contains a blood vessel that provides necessary nutrients to the cells of the hair follicle. The production of the hair itself and the formation of the hair matrix are carried out by keratinocytes. Pigmentation for the hair is provided by melanocytes.

Physiological Functions and Tissue Repair Processes

The skin plays a vital role in vitamin D production, as exposure to UV light encourages the body to produce this essential nutrient. Sensation is managed through nerve endings situated in both the dermis and the stratum basale. When the skin is injured, it undergoes specific recovery processes. Regeneration occurs when normal function returns to the tissue; this happens as stratum bacale cells reach contact with one another before fibroblasts have the opportunity to fill the area with scar tissue. Conversely, fibrosis is the process by which normal functioning tissue is replaced by scar tissue.

Clinical Classification and Characteristics of Burns

Burns are medically classified based on their severity and the depth of tissue damage. First Degree burns are limited to the epidermis and are characterized by symptoms of redness, pain, and swelling. Second degree burns are more extensive, involving both the epidermis and the dermis; these present with redness, pain, swelling, and the formation of blisters. Third degree burns are the most severe, involving damage to the epidermis, the dermis, and the hypodermis.

Diagnostic Procedures in Dermatology

Several methods are utilized to evaluate skin health and diagnose conditions. A skin biopsy involves the removal of a piece of tissue or a sample of cells from the body for examination. Skin scraping is a technique where skin cells are taken by scraping so they can be viewed under a microscope. Additionally, a wood light, which utilizes a black light, is used to visualize changes in skin pigment.

Pathologies: Skin Cancer and Infectious Diseases

Skin cancer is noted as the most common form of cancer and is heavily associated with exposure to the sun. Basal cell carcinoma is a type that generally tends not to metastasize. Squamous cell carcinoma originates from the keratin ocytes located in the stratum spinasum. Malignant melanoma is the rarest but most deadly form of skin cancer, often beginning within a mole.

Infections of the skin can be caused by bacteria, viruses, or fungi. Impetigo is a bacterial infection caused by staphyloceus, featuring vesicles that eventually burst and crust over. Cellulitis is a deeper infection involving the dermis or the hypodermis. Viral skin infections include warts, which are growths caused by HPV; Herpes simplex virus (HSV), which causes small, painful blisters; and Herpes varicella zoster, which is the causative agent for chicken pox and shingles. Fungal infections include Tinea, commonly known as ringworm, which is spread through contact with an affected person.

Parasitic Infestations and Chronic Skin Disorders

Parasitic infections include Scabies, which results from an infestation of mites and presents as red bumps. Psoriaris is an autoimmune condition characterized by an overgrowth of skin cells and improper keratinization. Atopic Dermatitis, also known as Atopic Eczema, is an allergic reaction that can be caused by infection. Hives, or Urticaria, are often the result of an allergic reaction to triggers such as insect bites. Scleroderma is a condition involving the accumulation of excess connective tissue and excess collagen, often linked to chemical exposure.
