Renal Maturation in Neonates

  • Neonatal Kidney Immaturity: At birth, the kidney is immature with decreased perffusion pressure, glomerular filtration rate (GFR), and diluting/concentrating ability.

  • Fluid Management: Neonates have poor water conservation skills, resulting in intolerance to both fluid restriction and overload. High insensible losses occur primarily through evaporation due to body surface area to weight ratio and immature skin.

  • Sodium Handling: In the first few days, neonates are obligate sodium losers but later improve sodium retention capabilities. They also tend to excrete glucose in urine.

  • GFR Development: GFR improves significantly in the initial weeks but does not reach adult levels until 8-24 months.

  • Renal Tubular Function: Continues to mature post-birth, achieving full concentrating ability by approximately 2 years of age.
