Onset: When did the symptom start? What were you doing when it started?
Provocation/Palliation: What makes it worse? What relieves it?
Quality: What does the symptom feel like? (e.g., a dry cough without any mucus).
Region/Radiation: Where is the pain located?
Patient should point to the affected area.
Radiation refers to whether the pain spreads beyond the initial site (e.g., lower back pain radiating across the back).
Severity: Assess the severity of symptoms using a pain scale from 0 to 10.
0 indicates no pain, whereas 10 is the worst pain imaginable, akin to passing out from pain.
