"The nursing process is a systematic, problem-solving approach used by nurses to identify, diagnose, and treat human responses to health issues.

1. Assessment
  • Definition: Collection of subjective and objective data about the patient.

  • Activities:

    • Health history

    • Physical exam

    • Review of records

    • Interviewing the patient and family

2. Diagnosis
  • Definition: Analysis of the assessment data to identify patient problems and formulate nursing diagnoses.

  • Activities:

    • Interpret and analyze data

    • Identify patient strengths and problems

    • Formulate nursing diagnoses (actual or potential)

3. Planning
  • Definition: Developing a plan of care based on identified nursing diagnoses and patient goals.

  • Activities:

    • Set priorities

    • Establish goals and expected outcomes

    • Select appropriate nursing interventions

4. Implementation
  • Definition: Performing the nursing interventions identified in the plan of care.

  • Activities:

    • Providing direct and indirect nursing care

    • Documenting care provided

5. Evaluation
  • Definition: Assessing the effectiveness of the nursing interventions and the patient's progress toward achieving goals.
