Helminths Overview

  • Helminths are parasitic worms with separate sexes (males and females). They often use humans as definitive hosts.
Nematodes (Roundworms)

    • Infection Method: Eggs are recovered from the perianal region, typically observed in children.

  • Hookworms:
    • Necator americanus (New World Hookworm)
    • Infestation: Penetrates skin, causes ground itch.
    • Identification: Paired teeth for anchoring.
    • Ancylostoma duodenale (Old World Hookworm)
    • Similar Features: Found in Africa and Europe.
  • Strongyloides stercoralis (Threadworm)
    • Characteristics: Asexual reproduction without the need for eggs in stool.
    • Disease Transmission: Can be transferred via organ transplants, especially in AIDS patients.
Cestodes (Tapeworms)
  • Characteristics: Hermaphroditic, flat, segmented worms.
  • Examples:
    • Hymenolepis nana (Dwarf Tapeworm): Moderate size, thickening filaments with hooks and suckers.
    • Taenia saginata (Beef Tapeworm): 15-30 lateral branches; humans are definitive hosts.
    • Taenia solium (Pork Tapeworm): Features a central hook, eggs indistinguishable from Echinococcus granulosus.
    • Diphyllobothrium latum (Fish Tapeworm): Eggs recovered in feces; associated with macrocytic anemia.
Trematodes (Flukes)
  • Characteristics: Non-segmented flatworms, often hermaphroditic, with snails as intermediate hosts.
    • Examples:
    • Fasciolopsis buski: Large intestinal fluke, eggs found in feces.
    • Fasciola hepatica: Sheep liver fluke, transmitted via contaminated water.
    • Clonorchis sinensis: Found in fresh water fish, linked to biliary cancer.
    • Paragonimus westermani: Lung fluke infecting humans through consumption of raw crab.
Definitions
  • Cercariae: The final larval stage of development in snails that becomes motile.
  • Metacercariae: Dormant stage in intermediate hosts.
  • Miracidium: Larval stage that emerges from eggs in fresh water.
  • Schistosomule: Stage when cercariae penetrates skin and loses its tail.

Protozoa and Their Diseases

Blood Protozoa
  • Plasmodium spp. (causing malaria)
    • Plasmodium falciparum: Causes the most severe form of malaria, transmitted by the Anopheles mosquito. Major symptoms include high fever and hemolytic anemia.
    • Plasmodium vivax: Known for benign tertian malaria, can relapse; ring-form trophozoites larger than normal RBCs.
    • Plasmodium ovale: Similar to vivax, but associated with older RBCs.
    • Plasmodium malariae: Causes quartan malaria, no reinfection after initial outbreak.
    • Babesia microti: Transmitted by ticks. Characteristic Maltese cross formation in infected RBCs.
Infectious Nematodes and Their Hosts
  • Loa Loa (African Eye Worm) - vector: Chrysops fly.
  • Trichinella spiralis - from undercooked wild game, causes trichinosis.
  • Wuchereria bancrofti - causes Bancroftian filariasis, transmitted by various mosquitoes. Associated with elephantiasis.
  • Onchocerca volvulus (River Blindness) - transmitted by blackflies.

Arthropods and Their Associated Diseases

  • Arthropods can lead to blood loss, disease transmission, and hypersensitivity reactions.
  • Examples of common arthropods:
    • Lice: Cause itching, can be asymptomatic.
    • Fleas: Transmit bacteria leading to various diseases.
    • Ticks: Include species that transmit Lyme disease (Borrelia burgdorferi) and Rocky Mountain Spotted Fever.

Amoebas and Flagellates
