High-Yield Special Tests for NP Certification
Murphy's Sign
Procedure: Palpate RUQ beneath costal margin while patient takes deep breath.
Positive Result: Abrupt cessation of inspiration due to pain.
Indication: Acute cholecystitis with 97% specificity when combined with RUQ tenderness.
McBurney's Point Tenderness
Location: 1/3 distance from ASIS to umbilicus in RLQ.
Significance: Point tenderness suggests appendicitis.
Diagnostic Combination: Combine with Rovsing sign (RLQ pain with LLQ palpation) and psoas sign (pain with right hip extension) for diagnosis.
Rebound Tenderness (Blumberg's Sign)
Procedure: Press slowly into abdomen, then release quickly.
Positive Result: Sharp pain on release.
Indication: Peritoneal inflammation, suggests peritonitis, appendicitis, or other acute abdominal emergencies requiring urgent surgical evaluation.
Obturator & Psoas Signs
Obturator Sign: Flex right hip/knee, rotate internally – pain suggests pelvic appendicitis.
Psoas Sign: Patient lies on left side, extend right hip – pain indicates retrocecal appendix or psoas inflammation.
Fluid Wave Test
Procedure: Assistant places hand vertically at midline. Tap one flank while palpating opposite side.
Interpretation: Transmitted impulse indicates ascites (≥1500 mL present).
Additional Method: Shifting dullness detects smaller volumes (≥500 mL).
Pediatric Abdominal Assessment: Age-Specific Norms
Infant (0-12 months)
Findings:
Prominent abdomen normal due to weak musculature.
Liver edge palpable 1-2 cm below costal margin.
Umbilical hernia common, typically resolves by age 4.
Conditions to Note:
Pyloric stenosis: olive-shaped mass in epigastrium (4-12 weeks).
Intussusception: currant jelly stools, dance sign (RLQ emptiness).
Toddler/Preschool (1-5 years)
Normal Appearance: Pot-belly appearance normal until age 5.
Findings:
Spleen tip may be palpable in healthy children.
Breath-holding during exam is common; distract with toys.
Appendicitis Presentation:
Atypical: diffuse pain, with vomiting predominating.
Documentation Requirement:

School-Age (6-12 years)

Adolescent (13-18 years)

Paralytic Ileus vs. Mechanical Bowel Obstruction
Paralytic Ileus
Key Finding:
Absent or hypoactive bowel sounds distinguish ileus from mechanical obstruction.
Clinical Presentation:
Diffuse, constant abdominal distension and discomfort.
Minimal to absent bowel sounds upon auscultation.
Nausea, vomiting (less severe than obstruction).
No passage of flatus or stool.
Tympany throughout abdomen on percussion.
Common Etiologies:
Post-operative (peaks 24-72 hours after abdominal surgery).
Medications: opioids, anticholinergics, calcium channel blockers (CCBs).
Electrolyte imbalances: hypokalemia, hypomagnesemia.
Intra-abdominal infection or inflammation.
Mechanical Bowel Obstruction
Key Finders:
High-pitched, hyperactive, tinkling bowel sounds occur proximal to obstruction.
Clinical Presentation:
Colicky, cramping abdominal pain (wavelike pattern).
Hyperactive then absent sounds as obstruction progresses.
Bilious vomiting (early small bowel); feculent vomiting (late).
Absolute constipation in complete obstruction.
Localized distension, possible visible peristalsis.
Common Etiologies:
Adhesions (60% of small bowel obstructions due to prior surgery).
Hernias (inguinal, femoral, incisional, umbilical).
Malignancy, especially colorectal cancer.
Volvulus, intussusception, strictures (IBD).
Emergency Criteria:
Both conditions require urgent evaluation.
Red flags for immediate surgical consultation: fever, peritoneal signs, severe pain, hematemesis, shock, suspicion of strangulation or perforation.
Imaging (CT abdomen/pelvis) confirms diagnosis.
Acute Inflammatory Conditions Requiring Urgent Recognition
01 Acute Appendicitis
Progression: Classic pattern includes periumbilical pain migrating to RLQ (McBurney's point) within 12-24 hours.
Associated Symptoms: Anorexia, nausea, low-grade fever (100-101°F).
Signs: Positive Rovsing sign, psoas sign, obturator sign.
Laboratory Findings: Leukocytosis (10,000-18,000).
Action Required: Immediate surgical referral; risk of perforation increases after 24 hours.
02 Acute Cholecystitis
Symptoms: RUQ pain radiating to right scapula, especially postprandial (after fatty meals).
Test Findings: Positive Murphy's sign (inspiratory arrest with RUQ palpation).
Associated Symptoms: Fever, nausea, vomiting.
Laboratory Findings: Elevated white blood cell count (WBC), bilirubin, alkaline phosphatase.

03 Acute Pancreatitis
Symptoms: Severe epigastric pain radiating to back, often seen after alcohol or fatty meal.
Patient Positioning: Often leans forward for relief.
Associated Symptoms: Nausea, vomiting, abdominal distension.
Laboratory Findings: Lipase elevated >3x normal (more specific than amylase).
Signs: Grey Turner/Cullen signs indicating hemorrhagic pancreatitis.
Management: NPO, aggressive IV hydration, immediate hospitalization.
04 Diverticulitis
Symptoms: LLQ pain, fever, altered bowel habits in patient >40 years.
Examination: Palpable tender mass possible.

Imaging: CT abdomen shows colonic wall thickening, pericolic fat stranding, abscess.
Management:
Uncomplicated: Outpatient antibiotics (ciprofloxacin + metronidazole).
Complicated: Perforation, abscess, or fistula requires hospitalization and surgical consultation.
Peptic Ulcer Disease: Assessment & Complications
Clinical Presentation by Location
Gastric Ulcers:
Epigastric pain worsens by eating (30-60 minutes post-meal).
Weight loss common due to food aversion.
Higher malignancy risk; biopsy required.
Duodenal Ulcers:
Epigastric pain relieved by eating or antacids.
Pain occurs 2-5 hours postprandial or may cause nocturnal awakenings (2-3 AM).
More prevalent than gastric ulcers (4:1 ratio).
Etiology & Risk Factors
Infection Risk:
H. pylori infection responsible for 70% duodenal and 50% gastric ulcers.
Medication Risks:
NSAID use (dose and duration dependent).
Lifestyle Risks:
Smoking, alcohol, and physiologic stress (ICU settings, burns).
Zollinger-Ellison syndrome:
Gastrinoma leads to recurrent ulcers.
Diagnostic Approach
Techniques:
Upper endoscopy (EGD) for direct visualization, biopsy, and H. pylori testing.
Non-invasive H. pylori Testing:
Urea breath test (most accurate) or stool antigen.
Serology indicates exposure, not active infection.
Life-Threatening Complications
Hemorrhage (15% of cases):
Results in hematemesis (coffee-ground or bright red) and melena.

Risk Factors & Screening
Demographics:
Men >65 years with a history of smoking have a 5-7% prevalence of AAA.
Additional Risks:
Hypertension, atherosclerosis, family history, Marfan syndrome, and chronic renal disease.
Screening Recommendations:
USPSTF recommends one-time ultrasound screening for men 65-75 who have ever smoked.
Women generally not screened unless there is a strong family history.
Physical Examination Findings
Key Findings:
Pulsatile periumbilical mass on deep palpation (sensitivity 30-50% for <5cm AAA and 75% for >5cm).
Abdominal bruit may be present.
Caution: Normal exam does NOT exclude AAA; sensitivity is poor, especially in obese patients.
Avoid aggressive palpation if AAA is suspected.
Rupture: Catastrophic Emergency
Classic Triad of Symptoms:
Abdominal/back pain, pulsatile mass, hypotension (only 25-35% present with all three).
Severe tearing back/flank pain, syncope, shock.
Critical Fact: Mortality rate of 50% prehospital and 90% overall if a rupture occurs.
Immediate Action Required: Do NOT delay for imaging if AAA is suspected. Activate vascular surgery, establish IV access, and prepare for the OR.
Monitoring Guidelines:
AAA Size Monitoring:
3.0-3.9 cm: ultrasound every 3 years.
4.0-4.9 cm: ultrasound every 6-12 months.
5.0-5.4 cm: ultrasound every 3-6 months.
≥5.5 cm: vascular surgery referral for repair consideration.
Attention Required: Rapid expansion (>0.5 cm in 6 months) requires earlier intervention.
Ascites & Splenomegaly: Examination Techniques
Ascites Assessment
Physical Examination
Inspection Findings:
Bulging flanks, everted umbilicus, abdominal distension.
Inguinal/umbilical hernias common from increased pressure.
Diagnostic Techniques:
Shifting Dullness:
Percuss from umbilicus to flank with patient supine; note the level of dullness.
Roll patient to side and re-percuss; dullness shifts to dependent side.
Detects ≥500 mL fluid (85% sensitivity).
Fluid Wave:
An assistant presses hand vertically along the midline, taps one flank while palpating the opposite flank.
Requires ≥1500 mL (50% sensitivity, 95% specificity).
Splenic Enlargement Physical Examination
Percussion Techniques
Castell's Sign:
Percuss lowest left intercostal space at anterior axillary line during inspiration and expiration. Dullness on inspiration suggests splenomegaly.
Traube's Space:
Normally tympanitic area between 6th rib to costal margin, mid-axillary to mid-clavicular line; dullness indicates enlargement.
Palpation Techniques
Positioning: Patient supine or in right lateral decubitus position.
Methodology: Palpate from RLQ toward LUQ during deep inspiration; the spleen descends with inspiration.
Finding: Normal spleen not palpable.
Critical Red Flags Requiring Immediate Referral
Acute Abdomen/Peritonitis
Clinical Signs:
Rigid, board-like abdomen with rebound tenderness, guarding, and absent bowel sounds.
Indicates perforation, ruptured viscus, or severe inflammation.
Action Required: Emergent surgical evaluation is necessary; the patient often lies motionless to minimize pain.
Hemodynamic Instability
Symptoms:
Hypotension, tachycardia, altered mental status, and signs of shock in conjunction with abdominal pain.
Suggests ruptured AAA, ectopic pregnancy, massive GI hemorrhage, or sepsis.
Response Needed: Activate emergency response, establish IV access, and prepare for immediate transport to the OR or ICU.
Severe Distension with Obstruction
Symptoms:
Progressive abdominal distension, bilious/feculent vomiting, absolute constipation, severe colicky pain.
High-pitched bowel sounds or sudden silence may indicate dire conditions.
Action Required: Concern arises for bowel ischemia, strangulation, or closed-loop obstruction which requires emergent decompression and surgical intervention.
Pediatric Emergent Conditions
Symptoms:
Inconsolable crying with legs drawn up indicates intussusception.
Currant jelly stools, bilious vomiting in neonates may suggest malrotation with volvulus.
Testicular pain/swelling noted during torsion; require urgent pediatric surgical or specialist consultation.
Documentation Requirement: Important to document time of symptom onset, pain characteristics (location, radiation, quality, severity 0-10), aggravating/alleviating factors, associated symptoms, vital signs, detailed examination findings, clinical decision-making, and time of referral. Thorough documentation protects patients and providers.
