Recording Requirements for Poisons
Must maintain a Prescription Book for all sales/supplies of poisons in private facilities.
Required entries on the day of sale or supply:
Date of sale/supply and serial number of the entry (if applicable).
Name of active ingredients or name of proprietary medicine with quantity supplied.
Name and address of the patient.
Recording Requirements for Psychotropic Substances
A Prescription Register for Psychotropic Substances is required for recording sales/supplies.
Required entries on the day of sale or supply:
Date and serial number of the sale or supply.
Name and strength of psychotropic substance with quantity sold/supplied/administrated.
Name, identity card/passport number, and address of the patient.
Update balance stock for each supply of the psychotropic substance.
Entries must be in chronological order.
Corrections made via marginal note or footnote with the date of correction; no cancellations or deletions.
Record each strength/type of psychotropic substance in a separate register or section.
Format of Record Books
All record books must be bound.
Soft copy records must be printed daily if transactions occur and maintained as a bound book.
