  • Teeth possess specific characteristics of shape and alignment based on their position in the jaw.

Evolution of Fundamental and Preventative Curvatures

  • Evolution has led teeth to develop specific shapes with fundamental curvatures at predefined areas.
  • These curvatures serve various functions:
    • Prevents disease and damage.
    • Inhibits bacterial invasion and calculus build-up.
    • Disperses excessive occlusal trauma and biting forces.
    • Protects the gingiva and periodontium.

  • Modern human teeth exhibit successful characteristics of shape and alignment.
  1. Specific location and size of proximal contact areas of various teeth.
  2. Size and location of interproximal spaces formed by the proximal contact surfaces.

Proximal Contact Areas

  • Proximal contact areas are defined as:
    • Locations where the proximal surfaces of adjacent teeth touch each other.
    • Functions:
    • Prevent food from packing between teeth, thus protecting underlying gum tissue.
    • Provide support and anchorage to teeth.
  • Adjacent teeth share interproximal bone:
    • The distal root portion of the first tooth is supported by the mesial root portion of the second tooth.
  • Contact areas are located at the widest portions of mesial and distal surfaces:
    • The distal contact area of one tooth touches the mesial contact area of the tooth behind it.
  • Proximal contacts:
    • Do not cover the entire surface but occupy a considerable portion,
    • E.g. Mandibular 1st premolar.

Contact Point

  • A contact point is where the occlusal cusp of one tooth touches the occlusal portion of another tooth in the opposing arch.
  • Distinction:
    • Contact area refers to the broader surface contact, whereas a contact point is a precise point of interaction.

Interproximal Spaces

  • Definition:
    • Interproximal spaces are triangular-shaped (V-shaped) spaces formed between teeth by one side's bone and proximate surfaces and their contact area on the other side.
  • The apex of the triangular shape is formed by the contact area, while the sides make up the proximal surfaces, and the alveolar bone forms the base.
    • Provides substantial bone support and anchorage.
  • Interproximal spaces are usually filled with gingival tissue known as papillary gingiva or interdental papilla.
  • Cervical width versus occlusal width:
    • These spaces are wider cervically than occlusally, enhancing vascular support for the interdental bone and papillary tissue.

Cervical Embrasure

  • When gingival recession occurs, the interdental papilla and bone do not fully occupy the interproximal space, resulting in a void, termed a cervical embrasure (blunted papillae).
  • This can be a pathological consequence of periodontal disease and may provide a place for bacteria and food debris to accumulate.
  • The extent of the void increases with the loss of bone.

Embrasures (Spillways)

  • Embrasures are the spaces occlusal to the contact areas between teeth. Functions include:
    • Facilitating the passage of food around teeth, preventing it from being forced into the contact area.
    • Named based on their location:
    • Facial (buccal or labial)
    • Lingual
    • Incisal
    • Occlusal
  • Gingival embrasures occur when the interproximal space is not occupied by tissue or bone; they are located gingival to the contact area and are not typically present.
  • Purposes of embrasures include:
    • Moving food away from contact areas to prevent interproximal packing.
    • Dissipating and reducing occlusal trauma forces on the teeth.
    • Providing self-cleaning via the action of the tongue, cheeks, and lips against the smooth tooth surfaces.
    • Protecting gingiva from undue trauma while permitting slight stimulation through frictional massage.

Location of Contact Areas and Embrasures

  • Facial View:
    • Anterior teeth contact areas are closer to the incisal surfaces, whereas posterior teeth are nearer to the middle third.
    • Increased width of embrasures is noted in posterior teeth compared to anterior teeth.
  • Occlusal View:
    • Anterior contact areas centrally positioned between labial and lingual surfaces.
    • Posterior teeth have contact areas slightly buccal to the center of the teeth.
    • Lingual embrasures are wider than facial embrasures due to narrower lingual sides.

Facial and Lingual Contours

  • Contours direct food off the teeth, influencing the suitable amount of frictional massage for the gingiva:
    • Correct facial or lingual curvature allows proper deflection of food, stimulating tissue and safeguarding gingival crevices.
    • Excess curvature can prevent adequate cleaning of gingiva due to improper food deflection direction.
  • General location of buccal heights:
    • Anterior teeth at the cervical third.
    • Lingual height for anterior teeth also at the cervical third.
    • For posterior teeth, proximal surfaces follow similar contour logic.

Curvature of the Cementoenamel Junction (CEJ)

  • Amount of curvature varies; anterior teeth show more curvature than posterior teeth.
  • Key points:
    • The anterior teeth have greater CEJ curvature with mesial curvature typically larger than distal.
    • Posterior teeth have less variation.
    • The normal curvature range: approximately 1mm less on the distal surface than on the mesial surface.
    • Example:
    • A maxillary incisor may have a 3.5mm mesial curvature CEJ and a distal curvature of 2.5mm.

Self-Cleaning Qualities of the Teeth

  • Teeth are inherently self-cleaning due to smooth enamel surfaces that reduce bacteria accumulation by allowing food substances to slip off easily.
  • The shape of teeth corresponds to their functions:
    • Canines act as piercing tools.
    • Premolars deflect food onto their occlusal surfaces for grinding.
  • Food processing aided by tongue and cheeks occurs throughout the interdigitation of molars to ensure effective swallowing.
  • Pits and fissures on occlusal surfaces:
    • Assist in dissipating occlusal forces arising during food grinding.
    • Techniques such as odontectomy or odontoplasty improve cleanliness by modifying pits for easier maintenance.

The Periodontium

  • Definition: The periodontium comprises supportive tissues adjacent to the teeth, including:
    • Free gingiva
    • Attached gingiva
    • Alveolar mucosa
    • Cementum, periodontal ligament, and bone.
  • These structures are vital for tooth support and anchorage.
  • An optimal angle of food deflection is critical for maintaining hygiene and preventing the exacerbation of periodontal issues:
    • Extreme angles may prevent proper tissue cleaning, leading to gingival disease.
  • Open contacts can lead to food accumulation in interproximal spaces resulting in periodontal tissue breakdown, while excessive spacing can permit food to quickly exit from areas without retention.
