Permit-Required Confined Space: Roles & Duties

Authorized Entrants ($1926.1208$)

• Know & understand space hazards (modes, signs/symptoms, consequences).
• Obtain hazard info via site-specific training, entry supervisor, and permit review.
• Correctly use assigned equipment.
• Maintain regular communication with attendant; report any danger or prohibited condition.
• Exit immediately when:
  • Ordered by attendant or entry supervisor
  • Hazard/prohibited condition is recognized
  • Evacuation is ordered.

Attendants ($1926.1209$)

• Know hazards, signs/symptoms, behavioral effects; consult SDS for materials.
• Keep accurate entrant roster; maintain continuous watch unless properly relieved.
• Communicate regularly with entrants; monitor well-being.
• Order evacuation when:
  • Prohibited condition arises
  • Entrant shows exposure effects
  • External danger develops
  • Unable to perform all duties safely.
• Prevent unauthorized entry: warn, stop, and report any unauthorized person.
• May perform non-entry rescue (lifeline pull-out).
• Summon rescue/emergency services promptly when needed.
• Perform no other tasks that distract; never leave post or enter space unless relieved.

Entry Supervisor ($1926.1210$)

• Know hazards & overexposure signs/symptoms; brief entrants and attendants.
• Before signing permit, verify:
  • Tests completed
  • Procedures & equipment in place
  • Rescue services available; communication adequate.
• Remove unauthorized entrants.
• Ensure acceptable conditions throughout entry; suspend entry if unsafe/unknown conditions arise.
• Terminate entry and cancel permit when work is complete or a non-correctable hazard is detected.

Role Overlap & Transfer (definition $1926.1202$)

• Person may serve as entry supervisor and entrant or attendant simultaneously if fully trained, equipped, and qualified.
• Supervisor role may be transferred during entry; incoming supervisor must reconfirm permit compliance and sign the permit.
