  • step 1: begin with a statement about the length, setting, and purpose of the session

    • introduction statement that debriefs where, what, how long, and why

  • step 2: provide a brief overview of client’s key difficulties

  • follow opening statements with a summary of the client’s performance and skilled interventions

    • can be sorted categorically or chronologically

      • chronologically: provides play by play of intervention session in the order in which events happened

      • categorically: can be divided into client factors and performance skills

  • use professional, concise and specific language

    • does not need to be written in complete sentences

    • should be written as concise as possible

    • may be helpful to display information in a chart

    • tips to increase professionalism

      • focus on occupation

      • focus on client’s response to treatment

      • write from client’s POV

      • be specific about assist needs

      • avoid judging the client

      • use standard abbreviations only

      • de emphasize the treatment media and instead focus on client factors and performance skills
