Introduction to Pathophysiology

Pathophysiology is the study of functional or physiologic changes in the body resulting from disease processes that alter normal body function and homeostasis.

  • Homeostasis: Maintenance of a stable internal environment despite external changes. Disrupted by aging, environment, genetics, and lifestyle choices.

  • Risk Factor: Anything that increases a person's chances of developing a disease.

  • Clinical Cues:

    • Observed Cues: Visible signs such as skin rash, swelling, or coughing.

    • Environmental Cues: Exposure to air pollution, lead paint, or contaminated water.

    • Historical Cues: Family genetic history, past surgeries, or medical history.

    • Time-Sensitive Cues: Critical signs requiring immediate action (e.g., stroke signs, airway obstruction).

Key Terminology
  1. Etiology: Cause or set of causes of a disease process.

  2. Idiopathic: A condition or disease with an unknown cause.

  3. Acute vs. Chronic:

    • Acute: Sudden onset, severe, short duration.

    • Chronic: Long-term, gradual onset, persistent condition.

  4. Manifestations: Clinical evidence of disease, composed of:

    • Signs: Objective indicators observed or measured (e.g., fever, rash, blood pressure).

    • Symptoms: Subjective experiences reported by the patient (e.g., pain, nausea, fatigue).

  5. Exacerbation: Period of increased severity of symptoms or disease.

  6. Complication: Secondary adverse outcome arising from a primary disease or treatment.

  7. Prognosis: Predicted forecast or outcome of a disease process.


Cellular Regulation and Adaptation

Cellular regulation includes all functions executed by cells to maintain homeostasis and respond to hormonal or cytokine signals.

Types of Cellular Adaptation
  • Atrophy: Decrease in cell size, leading to reduced tissue mass.

  • Hypertrophy: Increase in individual cell size, causing enlarged tissue mass.

  • Hyperplasia: Increase in the overall number of cells.

  • Metaplasia: Replacement of one mature cell type by another mature cell type (e.g., ciliated columnar epithelium replaced by stratified squamous epithelium in smoker airways).

  • Dysplasia: Abnormal development or disorganized growth of cells, varying in size and shape.

  • Anaplasia: Loss of cellular differentiation and structural orientation; characteristic of malignant neoplasms.

  • Neoplasia: Uncontrolled cellular proliferation resulting in "new growth" (tumors).

Cell Death Mechanisms
  • Apoptosis: Normal, programmed, and controlled cell death.

  • Necrosis: Unprogrammed, premature cell death resulting from acute injury, ischemia, or disease.

  • Gangrene: Tissue necrosis resulting from insufficient arterial blood supply.

Levels of Disease Prevention
  1. Primary Prevention:

    • Focuses on preventing the onset of disease entirely (e.g., vaccinations, healthy diet, environmental safety).

  2. Secondary Prevention:

    • Focuses on early detection, screening, and prompt intervention (e.g., mammograms, Pap smears, routine BP checks).

  3. Tertiary Prevention:

    • Focuses on managing established, complex diseases to prevent further disability and maximize quality of life (e.g., physical therapy post-stroke, rehabilitation programs).


Complications of Stress

Stress is a systemic response triggered by physical or psychological stressors that disrupt homeostasis.

General Adaptation Syndrome (GAS)
  1. Stage 1 – Alarm Stage:

    • Activation of the Sympathetic Nervous System (SNS) and adrenal glands.

    • Triggers the "fight-or-flight" response, releasing catecholamines and cortisol.

  2. Stage 2 – Resistance Stage:

    • Parasympathetic system attempts to compensate while hormone levels remain elevated to maintain body function.

  3. Stage 3 – Exhaustion Stage:

    • Energy reserves and physiological resources are completely depleted, leading to tissue damage and systemic vulnerability.

Effects of Prolonged Stress
  • Physiological Effects: Prolonged vasoconstriction, sustained cortisol elevation, suppressed immune response, and delayed healing.

  • Associated Systemic Disorders:

    • Gastrointestinal: Ulcerative colitis, peptic ulcers.

    • Nervous: Multiple sclerosis flare-ups, seizures.

    • Respiratory: Asthma exacerbations.

    • Cardiovascular: Hypertension, coronary artery disease.

    • Dermatologic: Eczema, acne.

Coping Mechanisms
  • Adaptive Coping: Beneficial strategies such as exercise, healthy nutrition, adequate rest, meditation, and counseling.

  • Maladaptive Coping: Harmful behaviors such as substance abuse, destructive habits, and poor decision-making.


Environmental Hazards

Environmental hazards alter cellular integrity and represent major public health risks.

Categories of Environmental Hazards
  • Chemical Hazards: Toxic substances, air pollution, industrial chemicals, and heavy metals (e.g., lead, mercury, hexavalent chromium).

  • Physical Hazards: Temperature extremes, ionizing/non-ionizing radiation, and excessive noise.

    • Heat Exhaustion: Characterized by excessive sweating, dizziness, rapid pulse, and pale, clammy skin.

    • Heat Stroke: A life-threatening emergency characterized by hyperthermia, absence of sweating, dry/red skin, confusion, and loss of consciousness.

  • Biological Hazards: Pathogenic bacteria, viruses, fungi (mold), parasites, allergens (pollen, dust), and vector stings/bites.

  • Food and Waterborne Hazards: Contaminated food or drinking water leading to systemic or gastrointestinal infections.


Cancer Pathophysiology

Cancer consists of a broad group of diseases characterized by uncontrolled cellular proliferation, abnormal tissue architecture, and loss of cell death pathways.

Pathophysiological Characteristics
  • Anaplastic, immature cells lacking cell-to-cell communication.

  • Altered membrane surface antigens.

  • Compression of surrounding blood vessels leading to ischemia and tissue necrosis.

  • Secretion of digestive enzymes that break down neighboring cellular matrices to facilitate direct invasion.

  • Secretion of growth factors to induce angiogenesis (formation of new blood capillaries).

  • Nutrient deprivation of normal host tissues.

The CAUTION Warning Signs
  • C: Change in bowel or bladder habits

  • A: A sore that does not heal

  • U: Unusual bleeding or discharge

  • T: Thickening or lump in breast or elsewhere

  • I: Indigestion or difficulty swallowing

  • O: Obvious change in a wart or mole

  • N: Nagging cough or hoarseness

Diagnostic Procedures
  • Biopsy: The only definitive diagnostic test for cancer classification.

    • Needle Biopsy: Minimally invasive procedure using a fine or core needle.

    • Endoscopic Biopsy: Performed during procedures like colonoscopy using an endoscope.

    • Surgical Biopsy: Excisional or incisional removal of tumor tissue (more invasive).

  • TNM Staging System:

    • T: Primary tumor size and local extent.

    • N: Regional lymph node involvement.

    • M: Distant metastasis presence (0 or 1).
