Notes on Librela/Solensia OA Monoclonal Antibodies
Overview
- Librela is a monoclonal antibody for dogs to treat osteoarthritis (OA) pain; Solensia is the analogous monoclonal antibody for cats. Both are approved for animals aged at least 1 year (dogs) and 12 months (cats).
- Both products directly target nerve growth factor (NGF) to reduce OA pain. Librela targets NGF in dogs; Solensia targets NGF in cats.
- NGF is crucial for development in newborns, but in OA its levels rise in the affected joint, contributing to pain and inflammation. The aim is to bring NGF levels in the joint back toward baseline to reduce pain signaling.
- Analogy: NGF in circulation is like dust in a room; Librela acts like a Roomba, sweeping NGF out of circulation so it cannot bind its receptor and propagate pain signals.
Mechanism of Action
- NGF binds to its receptor TRKA (TRK A) on inflammatory cells and within the pain/inflammatory cascade, contributing to pain perception.
- Librela/Silencia (Solensia) bind NGF, preventing NGF from binding to TRKA, interrupting the NGF-TRKA pain signaling pathway.
- When NGF cannot bind TRKA, the pain impulse is not sensitized and is less likely to reach the brain, reducing perceived pain in the pet.
- Contrast with Cytopoint: Cytopoint targets IL-31 (the itch driver in dogs); Librela/Solensia target NGF to address OA pain.
- Practical visualization: NGF roaming in the joint is vacuumed up by Librela, reducing the inflammatory cascade and pain signals.
Indications and Age
- Indications: OA pain relief in dogs (Librela) and cats (Solensia).
- Age requirements: dogs ≥ 1 year; cats ≥ 12 months.
- Contraindications discussed in practice include young animals (NGF involved in nervous system development) and pregnant/nursing animals due to NGF's role in development.
