Overview of the Shoulder Region
- The shoulder is formally defined as an area or a region of the body, rather than a single specific bone.
- There is no single "shoulder bone"; instead, the region is composed of a complex arrangement of bones and joints.
- The shoulder region includes 3 primary bones:
- The humerus: The proximal bone of the arm.
- The scapula: Formally known as the shoulder blade.
- The clavicle: Formally known as the collarbone.
The Clavicle: Anatomy and Orientation
- General Positioning: The clavicle is oriented to lay flat in the body, rather than vertically or at an angle.
- Structural Ends:
- Sternal End: This is the flat end of the bone. It is positioned medially and attaches to the sternum.
- Acromial End: This is the curved end of the bone. It is positioned laterally and attaches to the acromial process of the scapula.
- Key Landmark: There is a specific bump known as a tubercle (conoid tubercle) located on the bone.
- Right and Left Identification Criteria: To determine the side of the body the bone belongs to, you must align three factors simultaneously:
- The curved (acromial) part must face anteriorly (toward the front).
- The tubercle (bump) must face down (inferiorly).
- The flat (sternal) end must point medially toward the midline of the body/sternum.
The Scapula: Landmarks and Identification
- Glenoid Cavity: A flat, rounded surface where the head of the humerus articulates or connects. For proper anatomical orientation, the glenoid cavity must face laterally to meet the arm.
- Anterior Features:
- Subscapular Fossa: This is the large, relatively flat, shallow depression on the anterior (front) surface of the scapula.
- Posterior Features:
- Spine of the Scapula: A prominent ridge of bone found on the posterior side.
- Supraspinous Fossa: The smaller area of bone located superior to the spine.
- Infraspinous Fossa: The larger area of bone located inferior to the spine.
- Projections/Processes:
- Acromial Process (Acromion): Located at the terminal end (the tip) of the spine of the scapula. This is the site where the acromial end of the clavicle attaches.
- Coracoid Process: An anterior projection that is separate from the spine. It can be distinguished from the acromion by its position; while the acromial process is at the end of the spine, the coracoid is the "other" major projection.
The Humerus: Features and Orientation
- Anatomy and Joints: The proximal bone of the arm that articulates with the scapula to form the shoulder joint.
- Anterior vs. Posterior Identification: Look at the distal end of the bone. If you can see three distinct rounded structures (capitulum and the two parts of the trochlea), you are looking at the anterior side. On the posterior side, these specific structures are not visible in the same way.
- Proximal Landmarks:
- Head of the Humerus: The large, rounded proximal end. To articulate with the glenoid cavity, it must face medially.
- Greater Tubercle: The larger, higher bump located on the proximal end.
- Lesser Tubercle: The smaller bump located next to the greater tubercle.
- Intertubercular Groove: The literal groove or furrow located between the greater and lesser tubercles.
- Distal Landmarks (Anterior View):
- Capitulum: A rounded structure located on the lateral side of the distal humerus.
- Trochlea: A structure that looks like two bones/circles but is one single bone part. It is located medially.
- Determining Lateral vs. Medial: The side of the bone with the head is always the medial side. Therefore, the capitulum is lateral, and the trochlea is medial.
- Right and Left Identification Criteria:
- The head of the humerus must face medially.
- The rounded structures at the distal end must face anteriorly.
Questions & Discussion
- Identification Methodology: The instructor warned students not to identify bones by assembling them (e.g., fitting the humerus into the scapula). On assessments, bones are presented in isolation, and students must be able to identify the side (left/right) using only the landmarks of that single bone.
- Student Identification Exercises:
