General Survey: Initial Client Appraisal

Introduction to General Survey

  • Definition: The initial appraisal of a client's overall presentation and behaviors.

  • Purpose: To note any body system issues that may require a more focused assessment.

  • Initiation: Begins the moment a healthcare provider first observes the client.

First Contact Protocols

  • Privacy: Essential to provide and maintain client privacy.

  • Hygiene & Safety: Perform hand hygiene and don appropriate Personal Protective Equipment (PPE).

  • Self-Identification: Clearly state your name and professional role.

  • Patient Identification: Use two identifiers to confirm the patient’s identity.

  • Personal Information: Inquire about assigned sex at birth (ASAB), gender identity, and preferred pronouns.

  • Procedure Explanation: Explain the actions you are going to take and address any questions the client may have.

  • Consent: Obtain explicit consent from the client before proceeding.

Therapeutic Communication

Verbal Communication
  • Elements: Includes the words spoken (vocalization) and the tone used in conversation.

  • Key Skills: Active listening, expressing empathy, demonstrating respect, and showing acceptance.

Nonverbal Communication
  • Role: Body language provides cues that can correlate with a client's truer feelings.

  • Expression: Communicates warmth, interest, and availability.

  • Concept: Refers to
