Metabolism refers to all the biochemical processes that occur within an organism to maintain life. It involves the conversion of food into energy, the building and breakdown of necessary molecules, and the regulation of cellular processes to sustain vital functions.
The Digestive System
Function: The digestive system plays a critical role in physically and chemically breaking down food products, facilitating their absorption and transport to cells in different parts of the body, ensuring that nutrients are available for energy, growth, and cellular repair.
Four Stages of Food Processing:
Ingestion: This is the initial stage where food is taken in through the mouth.
Digestion: In this stage, food is mechanically and chemically broken down into smaller molecules. Mechanical digestion begins with chewing in the mouth, while chemical digestion involves enzymes that dismantle food components.
Absorption: This process occurs primarily in the small intestine, where digested nutrients are absorbed into the bloodstream and delivered to cells for use.
Egestion: The final stage involves the removal of undigested waste products from the body, primarily through the rectum and anus.
Mouth
The digestion process begins in the mouth, the first part of the digestive tract.
Types of Digestion:
Chemical Digestion: Enzymes present in saliva, such as amylase, initiate the breakdown of carbohydrates even before food is swallowed.
Mechanical Digestion: This process involves chewing (mastication), where food is broken into smaller pieces, facilitating easier chemical processing.
Saliva Composition:
Saliva contains ptyalin (salivary amylase), which initiates carbohydrate digestion.
It also includes lysozyme and antibodies (IgA) that help combat bacterial infections in the oral cavity.
pH of the Mouth: The pH of saliva is approximately 7.2 (slightly alkaline), creating an optimal environment for enzyme activity.
Production: The human body produces about 1 liter of saliva per day, which aids in both the mechanical and chemical processes of mastication and swallowing.
Salivary Glands
The salivary glands are critical in producing saliva, which softens food and begins the digestion process, ensuring that food can be easily swallowed and further digested in the gastrointestinal tract.
Esophagus and Stomach
Esophageal Phase: This phase involves peristalsis, the rhythmic contraction of muscles in the esophagus that moves food from the throat to the stomach.
Stomach Functions:
The stomach serves as a reservoir for food, temporarily holding it before it enters the small intestine for further digestion.
It is the primary site for food breakdown, utilizing strong acids and enzymes to convert food into chyme.
Mucosal Lining Structure: The stomach is lined with simple columnar epithelium that produces mucus, providing a protective barrier against the gastric acid.
Cell Types in Gastric Glands:
Chief Cells: These cells produce pepsinogen, an inactive enzyme that is converted to pepsin in the highly acidic environment of the stomach, essential for protein digestion.
Parietal Cells: These cells secrete hydrochloric acid (HCl), a crucial component of gastric juice that activates pepsin and helps with the breakdown of food particles. This process is stimulated by the hormone gastrin.
Mucus Neck Cells: These cells secrete mucus to protect the gastric lining from the corrosive effects of stomach acid.
Intrinsic Factor: A glycoprotein necessary for the absorption of Vitamin B12; its absence can lead to pernicious anemia.
Enteroendocrine Cells: These cells release hormones like gastrin and histamine, which coordinate gastric activity and regulation.
Gastric Juice Production: The stomach produces 2-3 liters of gastric juice per day, which is later neutralized by bicarbonate secreted from the pancreas as food enters the small intestine.
Small Intestine
The small intestine is the principal site for the completion of food digestion and nutrient absorption, with a surface area significantly increased by villi and microvilli to enhance absorption efficiency.
Duodenum: This is the first segment of the small intestine, approximately 25 cm long, which functions as a mixing bowl receiving chyme from the stomach along with bile and pancreatic juices that aid in further digestion.
Hormones Secreted:
Secretin: This hormone reduces gastric secretion while increasing the secretion of pancreatic enzymes and bicarbonate to neutralize stomach acid entering the small intestine.
Cholecystokinin (CCK): CCK stimulates the secretion of pancreatic enzymes and promotes gallbladder contraction, which releases bile to aid in fat digestion. It also decreases gastric motility, allowing for proper digestion and absorption.
Pancreatic Enzymes:
Bicarbonate: This substance neutralizes gastric acid as it enters the small intestine, providing an optimal pH for enzyme activity.
Trypsin: An enzyme that digests proteins into smaller peptides.
Lipase: An enzyme that breaks down lipids into fatty acids and glycerol.
Pancreatic Amylase: This enzyme continues the digestion of carbohydrates, breaking them down into simple sugars.
Nucleases: Enzymes that digest nucleic acids into nucleotides.
Intestinal Enzymes: These include sucrase, lactase, and maltase, which are responsible for the final stages of sugar digestion, breaking disaccharides into monosaccharides.
Lower GI Tract
The lower gastrointestinal tract includes key areas such as the cecum and ascending colon, where the absorption of water, electrolytes, and certain vitamins occurs.
The ileocecal valve ensures that fecal matter does not return to the small intestine, maintaining a one-way flow through the GI tract.
Common Disorders
Stomatitis
Stomatitis refers to the inflammation of the oral mucosa, which can be caused by infections, irritants, or nutritional deficiencies.
Symptoms include pain, mouth ulcers, and dry mouth, significantly affecting the ability to eat or speak.
Management includes medications for pain relief, dietary modifications to ensure easy-to-swallow foods, and proper oral hygiene to prevent infections.
Esophagitis
This condition involves inflammation of the esophageal mucosa, often resulting from chronic acid reflux (GERD).
Symptoms typically include heartburn, difficulty swallowing (dysphagia), and chest pain.
Treatment usually involves lifestyle and dietary modifications, alongside medications that reduce acid production.
Gastritis and Peptic Ulcer Disease (PUD)
Gastritis is the inflammation of the stomach lining, which can lead to the development of peptic ulcers, often caused by Helicobacter pylori infection, NSAID use, or lifestyle stressors.
Common symptoms include abdominal pain, nausea, vomiting, and bloating.
Management involves a combination of medications, dietary modifications, and sometimes surgical interventions, particularly in severe cases.
Diverticulitis and Diverticulosis
Diverticulosis is characterized by the formation of diverticula in the colon, which may lead to diverticulitis when these pouches become inflamed or infected.
Symptoms of diverticulitis can include abdominal pain, changes in bowel habits, and fever.
Management focuses on dietary changes to increase fiber intake, antibiotics for infections, and in severe cases, surgical intervention may be necessary.
Gastroenteritis
Gastroenteritis is marked by inflammation of the gastrointestinal tract, often caused by infections from viruses or bacteria, leading to diarrhea, vomiting, and dehydration.
Treatment emphasizes rehydration through fluids and careful monitoring to prevent complications, especially in vulnerable populations like children and the elderly.
Intestinal Obstruction
Intestinal obstruction can occur due to physical blockages, such as tumors or strictures, or functional issues like paralytic ileus, wherein the intestines fail to function properly.
Symptoms can include severe abdominal pain, vomiting, distension, and absence of bowel sounds.
Inflammatory Bowel Diseases - Ulcerative Colitis & Crohn’s Disease
Inflammatory bowel diseases involve chronic inflammation of the gastrointestinal tract, leading to symptoms of persistent diarrhea, abdominal pain, and weight loss.
Management focuses on controlling inflammation, managing symptoms, and preventing complications, often through a combination of medications and, in some cases, surgical interventions.
Hernias and Hemorrhoids
Hernias occur when internal organs protrude through weak spots in the abdominal wall and may require surgical repair to prevent complications.
Hemorrhoids, which can be external or internal, involve swollen veins in the rectum or anus, managed through dietary changes, topical treatments for relief, and surgical options if severe or recurrent.
