- Essentiality: Many inorganic elements are vital for life but can be poisonous at high concentrations. This duality is governed by the principle of Paracelsus.
- Ancient Applications: Egyptians used Cu to sterilize water (3000BC), and the Chinese utilized Au in medicines (2500BC). Paracelsus pioneered mineral use (1600s) specifically Sb, As, and Mg salts.
- Modern Milestones: K[Au(CN)2] for tuberculosis; Salvarsan introduced by Elrich in 1910 for syphilis; Sodium vanadate for rheumatoid arthritis.
- Critical Roles:
- Cupper: Essential for iron metabolism and haemoglobin synthesis.
- Iron: Vital for haemoglobin formation and oxidative processes.
- Cobalt: Stimulates bone marrow for erythrocyte development.
- Manganese: Involved in fatty acid and ATP synthesis.
- Deficiency Symptoms:
- Iron: Pernicious anemia and immune system disorders.
- Zinc/Zine: Growth retardation, testicular atrophy, and skin lesions.
- Cupper: Artery weakness, liver disorders, and brain disease/heart disease in infants.
- Wilson's disease: Genetically inherited metabolic defect where copper accumulation causes liver disease and Kayser-Fleischer rings in the cornea.

- Wilson's Disease: Zinc acetate is used to hinder the absorption of inorganic coppers in the digestive system.

- Lithium:
- Use: Treats bipolar disorder (BD) via diagonal relationship with Magnesium; Li+ displaces Mg2+ in binding sites.
- Toxicity: Narrow therapeutic window; plasma levels >1.5mmol/L cause tremors, convulsions, and renal damage.
- Interactions: NSAIDs can increase Lithium concentration in blood by up to 60%.
- Sodium:
- Roles: Electrolyte for extracellular fluid, osmoregulation, and muscle contraction.
- Compounds: NaCl and NaHCO3 for depletion/dehydration. Sodium bicarbonate (Na2CO) used for renal tubular acidosis and as an antacid.
- Cathartics/Laxatives: Sodium carboxy methylcellulose, Na2SO4 (Glauber's salt), and sodium tartrate (Na2C4H4O6).
- Magnesium:
- Milk of magnesia: Mg(OH)2 suspension used as an antacid and osmotic laxative.
- Magnesium trisilicate (MgSi3O8): Antacid for peptic ulcers forming a protective silica precipitate.
- Epsom salt (MgSO4): Used as a purgative.
- Calcium:
- Medical uses: Calcium carbonate (CaCO3) as an antacid; Calcium gluconate (Ca(C6H11O7)2) in vitamin supplements; calcium salt of atorvastatin (Lipitor).
- Interactions: Chelates tetracycline and quinolone antibiotics, preventing absorption.
- Barium:
- Barium meal (BaSO4): Radio-contrast agent for X-ray visualization of the G.I.T. due to its high atomic number, water insolubility, and non-toxicity.
- Strontium:
- Strontium-89: Radioactive isotope emitting beta rays (half-life 50.5 days) used to alleviate metastatic bone cancer pain.
Group 13 and Group 14 Pharmaceutical Agents
- Aluminium:
- Adjuvants: Alum (KAl(SO4)2⋅12H2O), Al(OH)3, and AlPO4 used in vaccines to stabilize antigens and enhance immune response.
- Raft Formations: Al-alginate formulations (Al(OH)3, alginic acid, NaHCO3) treat GERD by forming a buoyant gel barrier in the stomach.
- Phosphate Binders: Al(OH)3 reacts in the stomach and precipitates as AlPO4 in the distal intestines to treat hyperphosphataemia.
- Boron: Boric acid (H3BO3) as an antiseptic/antifungal. Bortezomib (Velcade) is a proteasome inhibitor for multiple myeloma.
- Silicon (Group 14): Silylation enhances drug lipophilicity. Examples include Silabolin (anabolic), Silperisone (muscle relaxant), Sila-haloperidol (schizophrenia), and Sila-venlafaxine (antidepressant).
- Cisplatin ([PtCl2(NH3)2]): Neutral chemotherapeutic agent for ovarian and testicular cancer. It binds to DNA bases in a cis configuration, causing cross-linking and apoptosis.
- Carboplatin: Second-generation platinum drug; less nephrotoxic but more myelosuppressive.
- Iron: Stored as Ferritin (water-soluble Fe(III)) in the liver. Administered orally as FeCl2, FeSO4, or Fe(II) gluconate.
- Zinc: Polaprezine used for gastric mucosal repair; ZnO used in Calamine lotion for pruritus and in diaper rash ointments.
- Gold: Sodium aurothiomalate and Auranofin used for rheumatoid arthritis; Auranofin deposits Au(I) in lysosomes to inhibit degradative enzymes.
- Cobalt: Constituent of Vitamin B12 (cobalamin). Cyanide antidotes include hydroxocobalamin and dicobalt edetate. Co−60 used as a radiation source for cancer therapy.
- Lanthanum: Lanthanum carbonate approved for treating hyperphosphatemia in renal disease patients.
- Vanadium: Vanadyl sulfate (VOSO4) demonstrated effectiveness in lowering blood sugar in diabetic patients.
