Elbow, Forearm, Wrist, and Hand Anatomy and Injuries

Anatomy of the Elbow

  • Key Components:
    • Lateral Epicondyle
    • Medial Epicondyle
    • Humerus
    • Ulna
    • Radius
    • Joint Capsule
    • Ligaments: Radial Collateral, Annular, Ulnar Collateral

Assessment of the Elbow

  • History:
    • Past injury and mechanism of injury
    • When, where does it hurt?
    • Motions affecting pain
    • Pain characteristics: type, quality, duration
    • Any sound or unusual feelings
    • Duration of disability and presence of swelling
    • Previous treatments
  • Observations:
    • Look for deformities and swelling
    • Assess carrying angle: Cubitus Valgus vs. Cubitus Varus
    • Measure elbow flexion and extension
    • Check for hyperextension (Cubitus Recurvatum)
  • Palpation:
    • Assess epicondyles, olecranon, distal humerus, proximal ulna
    • Feel for soft tissues: muscles, tendons, ligaments

Prevention of Elbow, Forearm, and Wrist Injuries

  • Protective Measures:
    • Use protective gear to minimize injury severity
    • Limit repetitive motions in sports (e.g., baseball, tennis)
    • Employ proper mechanics and adequate equipment
    • Maintain strength, flexibility, and endurance

Recognition and Management of Injuries to the Elbow

  • Olecranon Bursitis:

    • Cause: Direct blow
    • Symptoms: Pain, swelling, tenderness
    • Care: Ice for acute, protect in chronic cases, possible aspiration
  • Elbow Sprains:

    • Cause: Hyperextension or valgus forces
    • Symptoms: Medial elbow pain, inability to grasp, tenderness over the MCL
    • Care: Rest, ice, and gradual rehab; Tommy John’s procedure for instability
  • Lateral Epicondylitis (Tennis Elbow):

    • Cause: Repetitive microtrauma to extensor muscles
    • Symptoms: Aching pain, weakness in wrist, decreased ROM
    • Care: RICE, NSAIDs, ROM exercises, proper mechanics
  • Medial Epicondylitis:

    • Cause: Repeated flexion; valgus torque
    • Symptoms: Pain with movement, tenderness, mild swelling
    • Care: Sling, rest, NSAIDs, possible brace
  • Osteochondritis Dissecans:

    • Cause: Bone degeneration due to blood supply impairment
    • Symptoms: Sudden pain, joint locking
    • Care: Surgery if locking persists
  • Ulnar Nerve Injuries:

    • Cause: Cubital Valgus, ulnar nerve dislocation
    • Symptoms: Paresthesia in fingers
    • Care: Conservative management, potential surgery
  • Elbow Dislocation:

    • Cause: Fall on outstretched hand
    • Symptoms: Severe pain, swelling, disability
    • Care: Immobilization and physician referral
  • Fractures:

    • Cause: Fall on flexed elbow
    • Symptoms: Pain, swelling, potential deformity
    • Care: Ice, sling, physician referral

Anatomy of the Forearm

  • Key Structures:
    • Ulnar and Radial positions, interosseous membrane
    • Muscles: Biceps Brachii, Brachialis, Flexor/Extensor groups

Assessment of the Forearm

  • History:
    • Assess cause, symptoms, swelling, treatment history
  • Observation:
    • Inspect for deformities and pain during motion
  • Palpation:
    • Tenderness, edema, and other abnormalities

Recognition and Management of Forearm Injuries

  • Contusion:

    • Cause: Direct contact, common at ulnar side
    • Care: RICE and protection with pads
  • Shaft Fractures:

    • Cause: Falls, direct blows
    • Symptoms: Audible crack, severe pain
    • Care: RICE, immobilization, physician referral
  • Colles’ Fracture:

    • Cause: Fall on extended wrist
    • Symptoms: Silver fork deformity
    • Care: Cold compress, splinting, physician referral

Anatomy of the Wrist, Hand, and Fingers

  • Key Structures: Phalangeal and carpal bones, tendons, and ligaments

Assessment of the Wrist, Hand, and Fingers

  • History:
    • Assess past injuries and pain characteristics
  • Observation:
    • Look for swelling, discoloration, and any postural deviations
  • Palpation:
    • Check for pain and deformities in bones and soft tissues

Recognition and Management of Wrist and Hand Injuries

  • Wrist Sprains:

    • Cause: Abnormal movement; most common wrist injury
    • Care: X-ray, RICE, splinting
  • Wrist Tendinitis:

    • Cause: Overuse
    • Care: Ice, NSAIDs, wrist splint
  • Carpal Tunnel Syndrome:

    • Cause: Median nerve compression
    • Symptoms: Tingling, weakness
    • Care: Rest, NSAIDs, possible surgical intervention
  • Scaphoid Fracture:

    • Cause: Compression injury
    • Care: Immobilization, potential long recovery due to blood supply issues
  • Mallet Finger:

    • Cause: Blow to fingertip
    • Care: Splinting for extended period
